Resumo: In Brazil, and in many other countries, the increase in incidence, prevalence and proportional mortality from chronic diseases leads to the necessity of strengthening the monitoring and control measures of these pathologies. Among chronic diseases, hypertension and diabetes mellitus stand out as important risk factors for other diseases and may result in huge social and economic costs. It is in this context that the Brazilian Ministry of Health is promoting the reorganization of health care at primary level for hypertension and diabetes mellitus. In the country, primary health care is based on the principles and guidelines of the Unified Health System, and develops through the Family Health Strategy. In this scenario, the Laboratory of Evaluation, Monitoring and Surveillance in Health - LamSaúde develops epidemiological research and evaluation of policies, programs and health services, with an emphasis on attention to chronic noncommunicable diseases, particularly hypertension and diabetes. The purpose of this chapter is to present the main studies conducted by the research group about the care network for patients with chronic noncommunicable diseases in Pernambuco state in northeastern Brazil.

A hipertensão arterial e a diabetes mellitus destacam-se entre as doenças crónicas não transmissíveis como importantes fatores de risco para outros agravos, podendo acarretar em maiores custos sociais e económicos. É no âmbito da Atenção Primária à Saúde que o Ministério de Saúde brasileiro está promovendo a reorganização da atenção à hipertensão arterial e à diabetes mellitus. No país, a atenção primária encontra-se na base dos princípios e diretrizes do Sistema Único de Saúde e desenvolve-se através da Estratégia Saúde da Família. Nesse panorama, o Grupo de Pesquisas do Laboratório de Avaliação, Monitoramento e Vigilância em Saúde - LamSaúde desenvolve pesquisas epidemiológicas e de avaliação das políticas, programas e serviços de saúde, com ênfase na atenção às doenças crónicas não transmissíveis, particularmente hipertensão e diabetes.
