O potencial curativo do Alo-SCT baseia-se na eliminação da doença neoplásica residual (resistente a protocolos de quimio e/ou radioterapia previamente administrados) e na substituição parcial ou completa do sistema imunitário do doente pelo do dador. Os resultados de um Alo-SCT dependem de diversas varíáveis, entre as quais se destacam o tipo e estado da doença subjacente, a idade e comorbilidades do doente, a fonte de células progenitoras, o tipo de dador selecionado, o regime de condicionamento aplicado e os cuidados pós-transplante.Tratando-se de um procedimento com morbimortalidade significativa, é fundamental o máximo controlo dos riscos a ele associados. A investigação científica dos diversos fatores que influenciam o sucesso de um Alo-SCT tem vindo a permitir, ao longo dos anos, que este seja oferecido a um maior número de doentes, com taxas de eficácia e segurança crescentes. Assim, é pertinente elaborar uma sistematização da informação mais atual relativa a esta abordagem terapêutica, com o objetivo final de proporcionar aos doentes o melhor acompanhamento possível.Com o intuito de consolidar estes conhecimentos, escrevi esta revisão, que constitui um auxiliar à prática clínica, com base em diversas fontes literárias fiáveis sobre o Transplante Alogénico de Progenitores Hematopoiéticos.The allogeneic hematopoietic progenitor cell tranplantation (Allo-SCT) is a therapeutic option increasingly being used for the treatment of several diseases, mainly malignant neoplasms of hematologic origin. The curative potential of the allogeneic transplant is based on the elimination of the residual neoplastic disease (which had been resistant to previously administered chemo and/or radiotherapy protocols) and on the partial or total replacement of the patient’s immunitary system by the donor’s. The outcome of an Allo-SCT depends on several variables, most importantly the type and state of the underlying disease, patient’s age and comorbidities, source of stem cells, type of selected donor, the preparative regimen used and the post-transplant care, among others. Being a procedure with significant morbidity and mortality, the maximum control of the associated risks is fundamental. Scientific research on the various factors which influence the outcome of an Allo-SCT has allowed, throughout the years, to offer this treatment to more patients with increasing rates of efficacy and security. Therefore, it is pertinent to elaborate a systematization of the most recent information about this therapeutic approach, with the final purpose of giving patients the best clinical accompaniment possible.
