Resumo: The authors report a case of a 72 years old male who was admitted into hospital with a 3-4 months history ofgeneral weakness, loss of appe­ tite and weight loss. One week prior to admissi­ on, the patient developed diarrhoea, that was associated with cognitive and gait impairment. On physical examination, the patient was disorien­tated in space and time, presenting a generali­zed flaccid muscle weakness, impairedgait and cervical lymphadenopathy. Complementary investigations confirmed the diagnosis of an acquired immunodeficiency syn­drome and cerebral toxoplasmosis. The brain CT scan showed right cerebral hemisphere lesions conditioning signs of "space ocupying lesions". The patient had a good response to treatment indicated by a signiflcant regression of his de­mentia as well as gait improvement. We hope to call attention, by presenting this case, to the importance of the diagnosis of AIDS as a cause of dementia in elderly patients.

We hope to call attention, by presenting this case, to the importance of the diagnosis of AIDS as a cause of dementia in elderly patients.Os autores apresentam o caso clínico de um doente de 72 anos de idade, com queixas de astenia, anorexia progressiva não selectiva e emagrecimento não quantificado com evolução de 3-4 meses. Uma semana antes do internamento, inicia diarreia a que se associam deterioração cognitiva e alteração da marcha, motivo pelo qual recorre ao Serviço de Urgência, tendo ficado internado. Observaram-se adenopatias cervicais peque­nas, num doente desorientado temporo-espaci­almente, com força muscular globalmente dimi­nuída, sendo difícil a marcha e a posição ortos­tática. Os exames auxiliares de diagnóstico vi­riam a revelar estar-se perante um síndrome de imunodeficiência adquirida e toxoplasmose ce­rebral com lesões expansivas intra-axiais pro­fundas e do hemisfério cerebral direito, com importante área de edema a condicionar sinais de efeito de massa. Efectuada a terapêutica para a toxoplasmose, houve melhoria clínica signifi­cativa, com regressão do quadro demencial e recuperação da marcha.
