Resumo: According to the Histological Typinf of Odontogenic Tumors of the World Health Organization, the central giant cell granuloma is a non-neoplastic bone lesion.Histologically, it consists of fibrous tissue containing multiple foci of hemorrhage and aggregations of multinucleated giant cells. Sometimes, it´s possible to observe trabeculae of woven bone, forming within the septa of mature fibrous tissue, tha may transverse the lesion. The authors present a case report of a 14 year-ld caucasian male. The intra-oral examination revealed the absence of the right mandibular canine anda a distal displacement of the mandibular incisors. Radiographically, a large unilocular mandibular radiolucency, extending apically to the mandibular incisors and reaching the left mandibular canine, was observed associated with the right mandibular canine in an impacted position. The lesion was surgically removed, under general anesthesia. The clinical diagnosis was a dentigerous cyst arising from the right mandibular canine. Based on the histological examination, the lesion could be a central giant cell granuloma, or the brown node of hyperparathyroidism or cherubism. In order to achive a final diagnosis, serum chemistry results for calcium, phophorous and parathyroid hormone were requested. All the results were within the normal range.Based on the gathered information, the authors concluded it was a central giant cell granuloma.
