Estudo da influência da angiografia com fluoresceína bissódica nos parâmetros hemorreológicos de doentes com Diabetes Mellitus

Resumo: Chorioretinal angiography with fluorescein is an auxiliaiy method widely used in Ophthalmo logy which enables us to study the blood/humour and blood retina/choroideia interface. The presence of a strange compound in circulation may interfere with blood flow horneostasis. With the aim of studying fluorescein influence in blood rheology, blood samples were drawn before, immediately after and 30 minutes after fluorescein bolus injection; from 10 patlents with diabetes mellitus, with or without retinopathy, blood fluidity (hematocrit, erythrocyte aggregation and whole blood viscosity) and erythrocyte membrane functional properties were determined. After fluorescein injection there was erythrocyte hyperaggregation (37.70 o, p 0.004), was detected, hemogiobin concentration (p=O.O39) and hematocnt (p 0.0 13) decrease, and a double time increase of methemoglobin concentration (p<0.001) the erythrocyte membrane hydrophobic region became more rigid (p=O.Ol6). Ali these abnom~alities normaiized after 30 minutes. In conclusion, fluorescein angiography interferes acuteiy with the hemorheologicai parameters of patients with diabetes meilitus, with erythrocyte hyperaggregation which could interfere with the microcirculation of these patients.

A presença em circulação de uma substancia estranha ao organismo poderá influenciar a homeostasia do fluxo sanguíneo. Com o objectivo de estudar a influência da fluoresceína na reologia sanguínea colheu-se sangue antes, imediatamente apos e ao fim de 30 mm. Da injecção de fluoresceína em doentes com Diabetes Mellitus (n 10) com ou sem antecedentes de retinopatia diabética; estudou-se a fluidez sanguínea (hematócrito agregação eritrocitária e viscosidade sanguínea total) e a análise das características funcionais da membrana eritrocitária. Após a injecção da fluoresceína observou-se hiperagregação eritrocitária (37 700 p O 004) diminuição da concentração de hemoglobina (p 0,039) e do hematócrito (p 0,013) e houve duplicação da concentração de metahemoglobina (p 0,001). A viscosidade sanguínea total manteve se inalterada. A camada hidrofóbica da membrana eritrocitária tomou-se mais rígida (p 0016). Estas alterações normalizaram ao fim de 30 minutos.
