Resumo: | Introduction: A patient with hypertension (HTA) essential requires follow-up and reevaluation. At any time, the presence of alarm signals should increase the suspicion of secondary HTA. This article describes a case of renal parenchymal neoplasia as a cause of secondary HTA in the elderly with long-standing essential HTA. Case description: male, 89. In the last HTA follow-up visits and after optimization of the therapeutics and reinforcement of healthy lifestyle habits, it persistently presents blood pressure values in the order 140-160 / 70-80 mmHg and AMPA values in the order 155-185 / 70-80 mmHg. The user is compliant with his usual medication and shows himself to be quite concerned. Analytically unchanged again. Repeat echocardiogram and adrenal and adrenal echography were proposed. From the study conducted, the objective was a complex nodularity in the left kidney. For further clarification, it was proposed to perform renal Computed Tomography, which revealed probable renal parenchymal neoplasia. Comment: This case reflects the importance of valuing the concerns of asymptomatic elderly users, taking into account their health context and capacity to manage their disease, as well as to warn that the orientation of the secondary HTA study varies according to age group. |

A qualquer momento, a presença de sinais de alarme deve aumentar a suspeita de HTA secundária. Neste artigo descreve-se um caso de neoplasia do parênquima renal como causa de HTA secundária no idoso com HTA essencial de longa data. Descrição do caso: Homem, 89 anos. Nas últimas consultas de seguimento de HTA e após otimização da terapêutica e reforço dos hábitos de estilo de vida saudável, apresenta persistentemente valores de pressão arterial na consulta na ordem 140-160 / 70-80 mmHg e valores de AMPA na ordem 155-185 / 70-80 mmHg. O utente é cumpridor da sua medicação habitual e mostra-se bastante preocupado. Analiticamente sem alterações de novo. Propôs-se a realização de ecocardiograma de repetição e ecografia renal e suprarrenal. Do estudo dirigido efetuado, objetivou-se uma nodularidade complexa no rim esquerdo. Para melhor esclarecimento, foi proposto realização de Tomografia Computorizada renal que revelou provável neoplasia parenquimatosa renal.
