Sensibilidade de agentes quimioterápicos : proposta terapêutica

Resumo: | Um dos maiores desafios colocados às equipas de saúde é o tratamento de infecções provocadas por agentes bacterianos. A aquisição de resistências aos antibióticos mais utilizados tem registado um aumento significativo constituindo uma preocupação a nível mundial. Escherichia coli, Klebsiella pneumoniae e Pseudomonas aeruginosa representam os principais agentes ictiológicos das infecções comunitárias mais comuns: as infecções do tracto urinário. Mycobacterium tuberculosis causa anualmente milhões de mortes por tuberculose em todo o mundo, tendo associada taxas de morbi-mortalidade significativas e constituindo um problema de saúde pública global. Estudos recentes retratam o incremento de resistências, principalmente a nível secundário ou resistências adquiridas. A heterogeneidade de fenótipos da estirpe, parecem constituir um factor de protecção, influenciando na capacidade de a tentar combater. Este trabalho encontra-se estruturado em duas partes: numa primeira etapa procurou realizar-se uma análise do fenómeno resistência/sensibilidade a alguns antibióticos por parte das estirpes anteriormente referidas aos fármacos testados. Quanto às infecções urinárias, as amostras foram analisadas consoante a resistência a um qualquer antibiótico (testado) e distribuídas pelas estirpes a estudar: Escherichia coli, Klebsiella pneumoniae e Pseudomonas aeruginosa. No que concerne ao estudo dos casos de tuberculose, por Mycobacterium tuberculosis, os dados foram separados em contexto ambulatório e hospitalar. É de interesse referir que os dados obtidos retratam uma região específica do norte de Portugal, não podendo ser generalizados a nível nacional. Numa segunda fase pretendeu-se descrever a fagoterapia, com recurso a bacteriófagos, como uma proposta terapêutica viável ao uso de antibióticos. ABSTRACT: The treatment of infections caused by bacterial agents is one of the biggest problems to healthy teams. The resistance acquired by the microorganisms to the most used antibiotics has been registering a significant raise and represent a general preoccupation. Escherichia coli, Klebsiella pneumoniae and Pseudomonas aeruginosa were the most common etiologic agents for communitarian infections like the urinary tract infections. Mycobacterium tuberculosis is responsible for millions of deaths by tuberculosis, all around the world. At this pathogen we can associate a significant incapacity and mortality number that traduce a global health problem. Recent studies describe a raise in resistance cases, on a secondary level or even in acquired resistances. The heterogenic fitness of this group is associated to the difficulty to destruct him. This work was structured in two parts: in a first place we try to analyse the resistance/sensibility occurrence to some antibiotics tested, by bacterial groups referred. The urinary tract infections cases were selected by the presence to one antibiotic resistance tested and distributed by one of these bacterial group in study: Escherichia coli, Klebsiella pneumoniae or Pseudomonas aeruginosa. In tuberculosis cases studied by Mycobacterium tuberculosis, we made the separation into ambulatory and hospitable cases. Is relevant to say that the results represent a specific area from Portugal and they can’t be extended into a national level. In a second place we tried to describe bacteriophage therapy, recurring to Bacteriophages, like a viable therapeutical alternative to antibiotics. |
