Os estudos científicos têm um grau de divergência elevado, que não é possível garantir, cientificamente, que a crioterapia tenha feitos terapêuticos na pessoa com joelho submetido a uma AT. Objetivo: O estudo teve como principal objetivo, avaliar o efeito da crioterapia sobre a temperatura superficial cutânea (TSC) do joelho da pessoa submetida a AT. Metodologia: reproduziu-se um desenho de investigação baseado no modelo em uso de várias unidades de saúde de Portugal, que envolve a aplicação da crioterapia, na ATJ, no pós-operatório (três dias). Trata-se de um estudo de natureza quasi experimental, fatorial 1X4X6: uma modalidade de crioterapia (saco de gelo) versus quatro interfaces (ligadura Robert Jones modificada (LRJ); ligadura de malha tubular de algodão jersey (MT); pano fino de algodão (PA); compressa não tecido desdobrada 15X20cm (CNT) versus seis períodos de aplicação da crioterapia (10, 20, 30, 40, 50 e 60 minutos). A amostra foi constituída por 60 participantes, distribuídos por um processo de amostragem de conveniência, com distribuição aleatória dos participantes pelos 4 grupos de intervenção (interfaces). A principal medida de resultado foi a TSC do joelho submetido a AT, avaliada através de uma sonda termopar de superfície. No estudo, no total, foram realizadas 720 aplicações de crioterapia e 2880 monitorizações da TSC. Resultados: no grupo que usou como interface a LRJ, os resultados demonstram ínfimas diminuições da TSC (0,38°C) do joelho submetido a AT, após a aplicação da crioterapia. A TSC média, após 180 aplicações de crioterapia, foi de 34,13 °C, considerada a temperatura basal dos tecidos, não se observando o efeito térmico. Consequentemente, não foram observados os efeitos terapêuticos da crioterapia. Nos outros três grupos em estudo (MT, PA e a CNT) as descidas das TSC do joelho, após a crioterapia, foram evidentes em todos os períodos de aplicação de crioterapia. A MT desceu 10,29°C, o PA 11,64°C e a CNT 12,46°C, consubstanciadas por TSC médias de MT (M = 23,90°C), PA (M = 22,84°C) e CNT (M = 21,92°C). Nestes grupos foram evidenciadas diferenças estatisticamente significativas (p < 0,001) na diminuição da TSC da face lateral e medial do joelho submetido a AT, em todos os períodos de aplicação da crioterapia. Estes resultados não significaram a obtenção de valores da TSC considerados terapêuticos (≤15°C). Os três grupos ficaram no padrão térmico designado de subótimo. A avaliação do impacto terapêutico (NNT) da crioterapia no joelho submetido a AT foi nulo, com valores nos grupos da LRJ e da MT (NNT = 0), no grupo do PA (NNT=45) e no grupo CNT (NNT=30). Conclusão: nas condições experimentais em estudo, a crioterapia aplicada com saco de gelo no joelho submetido a artroplastia, usando como interface a LRJ, não teve nenhum efeito terapêutico. Quando a crioterapia foi aplicada, usando como interface a MT, é muito improvável serem observados efeitos terapêuticos da crioterapia. Nos grupos da interface de pano e da CNT, poderão, esporadicamente, observar-se alguns efeitos terapêuticos da crioterapia, mas sem a frequência, robustez, magnitude e intensidade desejada. Este será um dos principais fatores que contribuirá para a existência de evidência cientifica conflituante na aplicação da crioterapia na pessoa com o joelho submetido a artroplastia total.Context: The state of scientific knowledge inherent to the application of cryotherapy on the knee with total artroplasty (TA) in the postoperative period is surrounded by conflicting scientific evidence. Scientific studies have a high degree of divergence, which turns impossible to scientifically guarantee that cryotherapy has therapeutic effects in the person undergoing total knee arthroplasty (TKA). Objective: The main goal for this study was to evaluate the effect of cryotherapy on the surface skin temperature (SST) of the knee from a person undergoing TKA. Methods: A research design was reproduced based model in use by several health units in Portugal, which involves the application of cryotherapy, in TKA, in the postoperative period (three days). This is a quasi-experimental study and 1x4x6: one cryotherapy modality (ice bag) versus four interfaces (modified Robert Jones bandage – RJB; jersey cotton tubular bandage – TB; fine cotton cloth – CC; non-woven swabs 15X20cm NWS – unfolded) versus six periods of criotherapy application (10, 20, 30, 40, 50 and 60 minutes). The sample consisted of 60 participants, distributed through a convenience sampling process of 15 participants in 4 intervention groups (interfaces). The main outcome measure was the SST of the knee with TA using a surface thermocouple probe. In this study, it was performed 720 applications of cryotherapy and 2880 SST monitoring of the knee with TA. Results: In the group using the RJB interface, the statistical data showed minimal decreases on the SST (0.38°C) of the knee with TA, after the application of cryotherapy, the average SST after 180 applications of cryotherapy was 34.13°C, considering the tissues basal temperature, without observing the thermal effect, consequently, the therapeutic effects of cryotherapy were not observed. In the other three groups, studying TB, CC and NWS, the decrease in the knee SST was evident in all periods of cryotherapy application. The TB dropped 10.29°C, the CC 11.64°C and the NWS 12.46°C. Substantiated by average SST of TB (M = 23,90°C), CC (M = 22,84°C) e NWS (M = 21,92°C). In these groups, statistically highly significant diferences were evidenced (p < 0,001) in the decrease of the lateral and medial region of the knee with TA’s SST, in all periods of application of cryotherapy (Wilcoxon test). These results did not mean the therapeutic SST values were achieved (≤15°C). The three groups were within the suboptimal thermal pattern. The Number Needed to Treat (NNT) of cryotherapy on the knee with TA was null in the RJB and TB group the (NNT = 0) in the CC group the (NNT = 45) and in the NWS group the (NNT = 30). Conclusions: Under the experimental conditions under this study, the cryotherapy applied with na ice bag to the knee with total arthroplasty using the RJB as na interface had no therapeutic effect. When cryotherapy has been applied using TB as an interface, therapeutic effects of cryotherapy are very unlikely to be observed. In the CC and NWS interface groups, some therapeutic effects of cryotherapy may sporadically be observed, but without the desired frequency, robustness, magnitude and intensity.
