Resumo: | Many sports activities contain jumping and landing component exposing athletes to high joint loads. Consequently, these athletes have a high incidence of acute and overuse injuries such as anterior cruciate ligament (ACL) rupture and patellar tendinopathy (PT), respectively. In jumping athletes, the incidence of PT ranges from 11.8% to 50% according with sport level and what kind of sport. Regarding ACL injuries, up to 121 injuries per 100,000 people/year. Athletes who undergo ACL reconstruction are subject to second ACL injury or knee osteoarthritis. Considering the possible influence of biomechanical factors in these injuries, the objectives of this thesis was to verify factors associated with knee joint overload during landing phase of single leg drop vertical jump and its implications for rehabilitation of PT and for athletes with ACL reconstruction. In this thesis, two studies were carried out. In the first study, the objective was to evaluate the association between peak ankle dorsiflexion and ankle dorsiflexion excursion during landing with peak vertical ground reaction force (vGRF), vGRF load rate, peak patellar tendon force, patellar tendon force load rate and peak ankle plantar flexor moment during landing phase of single-leg drop vertical jump task. In this way, a three-dimensional biomechanical analysis of 26 healthy recreational jumping athletes was performed using a camera system synchronized to a force plate. Ankle dorsiflexion excursion was negatively correlated with vGRF loading rate and positively with ankle plantar flexor moment. In addition, there was a positive correlation between ankle dorsiflexion peak and vGRF peak. In the second study, the objective was to compare the peak of the vGRF, peak moments, hip, knee and ankle moments in the peak of the vGRF and total support moment during landing phase of single-leg drop vertical jump between the involved and uninvolved limb of athletes with ACL reconstruction and heathy control athletes. For this purpose, using the same methodology of first study, 12 athletes with ACL reconstruction with hamstring ipsilateral grafts (GRLCA) and 14 healthy recreational control athletes (GC) were assessed. Peak of vGRF and knee extensor moment at peak of vGRF were higher in the involved limb of reconstructed athletes than in the control athletes. From these two studies, we concluded that: 1- lower ankle dorsiflexion excursion is associated with a greater vGRF loading rate, but there is no association between ankle kinematics and patellar tendon loads; and 2- Compared to the control group, the involved limb of the ACLRG presents greater vGRF and greater knee extensor moment at peak of vGRF during SLDVJ landing. These results suggest that greatest load occurs on the ACL graft at the time of the peak of vGRF. |

Por consequência, esses atletas possuem alta incidência de lesões agudas ou por uso excessivo, como rupturas do Ligamento Cruzado Anterior (LCA) e a tendinopatia patelar (TP), respectivamente. Em atletas saltadores a incidência da TP atinge de 11,8% a 50% dependendo do nível esportivo e esporte realizado. Em relação à lesão do LCA, tem sido relatada até 121 lesões a cada 100.000 pessoas/ano. Os atletas que são submetidos à reconstrução do LCA estão sujeitos à segunda lesão do LCA (do enxerto ou do LCA contralateral) e osteoartrite do joelho. Considerando a possível influência de fatores biomecânicos à ocorrência dessas lesões, o objetivo dessa tese foi verificar fatores associados a sobrecarga na articulação do joelho durante a fase de aterrissagem do single-leg drop vertical jump e suas implicações para a reabilitação na TP e de atletas com reconstrução do LCA. Para isso foram realizados dois estudos. No primeiro estudo o objetivo foi avaliar a associação entre o pico de dorsiflexão do tornozelo e a excursão de dorsiflexão do tornozelo durante a aterrissagem com o pico da força de reação do solo vertical (FRSv), taxa de carga da FRSv, o pico da força no tendão patelar, taxa de carga da força no tendão patelar e o pico do momento flexor plantar do tornozelo durante a fase de aterrissagem do single-leg drop vertical jump. Para isso, uma análise biomecânica tridimensional de 26 atletas recreacionais sadios saltadores foi realizada usando um sistema de câmeras sincronizado a uma plataforma de força. A excursão de dorsiflexão do tornozelo correlacionou-se negativamente com a taxa de carga da FRSv e positivamente com o momento flexor plantar do tornozelo. Além disso, houve correlação positiva entre o pico de dorsiflexão do tornozelo e o pico da FRSv. No segundo estudo o objetivo foi comparar o pico da FRSv, os picos dos momentos e os momentos do quadril, joelho e tornozelo no pico da FRSv, bem como o momento total de suporte, durante a fase de aterrissagem do single-leg drop vertical jump, entre o membro envolvido e não envolvido de atletas com reconstrução do LCA e com atletas sadios. Para isso, utilizando os mesmos métodos do primeiro estudo, 12 atletas com reconstrução do LCA com enxerto dos tendões flexores autógenos ipsilaterais (GRLCA) e 14 atletas controle sadios (GC) foram avaliados. O pico da FRSv e o momento extensor do joelho no pico da FRSv foram maiores no membro envolvido do GRLCA do que no GC. A partir desses dois estudos concluímos que: 1- a menor excursão de dorsiflexão do tornozelo está associada a uma maior taxa de carga da FRSv, mas não há associação entre a cinemática do tornozelo e as cargas no tendão patelar; e 2- Comparado ao grupo controle, o membro envolvido do GRLCA está sujeito a maior FRSv, bem como maior momento extensor do joelho no instante do pico da FRSv durante a aterrissagem do SLDVJ.
