Resumo: | Cardiopulmonary fitness assessment is a valuable resource to obtain quantitative indicators of an individual's physical performance. The cardiopulmonary exercise test (CPX), considered the gold standard test for this evaluation, is costly and difficult to be accessed for the general population. In order to make this evaluation more accessible, minimizing costs, and to better reflect the performance of daily life activities, alternative tests were proposed. Morbidly obese patients present limitations that limit physical performance assessment through CPX and could benefit from a test of shorter duration, provided it is validated. Aim: To validate the two-minute step test (2MST) in morbidly obese patients with or without associated comorbidities, in relation to the CPX. Methods: Observational study including 31 obese individuals (BMI>35kg/m²). The CPX and the 2MST were performed and metabolic and ventilatory parameters were recorded. Pearson correlation analysis and multiple linear regression analysis were performed to evaluate the CPX peak VO2 best predictors. Bland-Altman analysis was performed to quantify the agreement between the two methods. Results: CPX peak VO2 and 2MST peak VO2 were significantly correlated (r=0.70, p<0.001); the 2MST number of step cycles (UDS) was correlated with the distance covered during the CPX (r=0.39, p=0.03) and with the CPX peak VO2 (r=0.38, p=0.03). The Bland-Altman analysis showed satisfactory agreement between the methods. The reference equation was obtained: VO2(mL.kg-1.min-1)=21.888+(0.0874*total UDS)-(0.206*BMI)-(0.053*Age), with estimated standard error=1.75mL.kg-1.min-1. Conclusion: The 2MST is a viable, practical and easily accessible test for cardiopulmonary fitness evaluation. UDS, BMI and age can satisfactorily predict peak VO2. |

O teste de exercício cardiopulmonar (TECP), considerado o teste padrão-ouro para esta avaliação, é oneroso e dificulta o acesso da população. A fim de tornar esta avaliação mais acessível, e visando refletir melhor o desempenho das atividades de vida diária, testes alternativos foram propostos para diferentes populações. Os pacientes obesos mórbidos apresentam limitações que dificultam e limitam as avaliações por meio das ferramentas existentes. Acredita-se que esta população pode ser beneficiada pela realização de um teste de menor duração, uma vez que este seja considerado válido. Objetivo: verificar a validade do teste de degrau de dois minutos (TD2) em pacientes obesos mórbidos ou com comorbidades associadas, em relação ao TECP. Métodos: Estudo observacional com uma amostra de 31 indivíduos obesos (IMC>35kg/m²). Foram realizados o TECP e o TD2 e foram registrados os parâmetros metabólicos e ventilatórios. Para análise dos dados foram aplicadas correlação de Pearson e análise de regressão linear múltipla para avaliação dos melhores preditores do VO2pico do TECP. Resultados: o VO2 mL.kg-1.min-1 no pico do TECP e o do pico do TD2 se correlacionaram significativamente (r=0,70; p<0,001). Adicionalmente, o número de ciclos subidas e descidas do degrau (SDD) do TD2 correlacionou-se com a distância percorrida (r=0,39; p=0,03) e com o VO2 mL.kg-1.min-1 obtido no TECP (r=0,38; p=0,03). Análise de Bland-Altman demonstrou concordância satisfatória entre os métodos. Deste modo, foi obtida a equação de referência: VO2(mL.kg-1.min-1) = 21,888+(0,0874*total SDD) - (0,206*IMC) -(0,053*Idade), com erro padrão estimado de 1,75 mL.kg-1.min-1. Conclusão: o TD2 é uma forma válida de avaliação da aptidão cardiopulmonar.
