Resumo: The thesis consisted of three studies described below. The Study I, entitled: Is there a chronic sleep stage-dependent linear and nonlinear cardiac autonomic impairment in obstructive sleep apnea? The aim of this study was to perform HRV analysis in apneafree samples as well as during stage 2 and rapid eye movement (REM) sleep in mild and moderate OSA (MiOSA andMOSA,respectively) subjects as well as health controls (NonOSA). Methods This study included 20 MiOSA (37±14 years), 20 MOSA (39±8 years), and 18 NonOSA (36±8 years) subjects. Subjects underwent in-laboratory overnight polysomnography with electrocardiography recording. HRV indices were obtained by analyzing the R R intervals (RRi) in 5-min apnea-free samples by the linear frequency domain [low frequency (LF), high frequency (HF) and LF/HF], Poincaré plot [standard deviation (SD1) and (SD2)], recurrence plot [mean line length (Lmean)], recurrence rate (REC), determinism (DET), and Shannon entropy (ShanEn). The MOSA group presented with higher LF, LF/HF and DET indices compared to NonOSA as well as a lower parasympathetic index (HF), suggesting sympathetic hyperactivity in MOSA subjects. Interestingly, MiOSA subjects failed to show the expected linear HRV difference between sleep stages, as observed in NonOSA, which may represent an early onset of autonomic impairment at this stage of OSA. In OSA patients, there is a chronic sleep stage dependent impairment of linear and nonlinear cardiac autonomic modulation. Interestingly, this impairment may be identifiable during the early stages of the disease. The Study II, entitled: Heart Rate Variability and Cardio-Respiratory Coupling during sleep in Patients prior to Bariatric Surgery The aim of this study was to determine the relationship among severity of obesity, autonomic cardiac regulation, respiratory variations, and cardio-respiratory coupling during different sleep stages (SS), using spectral analysis of HRV and respiration variability (RS) signals in patients pre-operative BS. 29 consecutive pre-operative BS and ten eutrophic subjects (controls) underwent polysomnography. The spectral and cross-spectral parameters of the HRV and RS were computed during different SS. Spectral analysis of the HRV and RV indicated lower respiration regularity during sleep, and a lower HRV in obese patients (OP) during all SS when compared with controls (p<0.05). Severely (SO) and super obese patients (SOP) presented lower values of LF/HF ratio and LF power during REM sleep and higher HF power (p<0.05), while morbidly obese (MO) presented lower LF/HF ratio and LF power in SS-S2 and higher HF power when compared to controls (p<0.05). The cross-spectral parameters showed that SOP presented lower % of tachogram power coherent with respiration in SS-S3 when compared to controls (p<0.05). Patients prior to BS presented altered HRV and RV in all SS. SO, MO and SOP presented altered cardiorespiratory coupling during sleep and these alterations are related with severity of obesity and OSA parameters. Our third study entitled "Contrasts of heart rate variability and cardio respiratory coupling in young and elderly patients with OSA", aimed to contrast the impact of OSAS between young and old in HRV and CRC. It also seeks to establish whether age impacts differently on HRV and CRC during different sleep stages using spectral analysis of HRV and RV in young and elderly patients with and without OSA. The spectral analysis of HRV showed that the presence of OSA negatively impacted on HRV youth, with higher values of LF/HF (p <0.05) during wakefulness and different sleep stages with lower values of LF/HF during S2 and REM sleep (p <0.05). We also observed that age negatively affected HRV during wakefulness and sleep in elderly with higher LF/HF during waking. Young and old with OSA had significantly lower % of tachogram power coherent with respiration only during wakefulness (p <0.05). The presence of OSA has a negative impact on HRV in young and elderly, with reduced autonomic modulation during waking and REM sleep. Age has a negative impact on HRV during wakefulness and sleep in the elderly. The presence of OSA and age affect negatively the ACR during wakefulness.

O estudo I, intitulado: Existe um prejuízo autonômico cardíaco linear e não linear, crônico e estágio dependente em pacientes com síndrome da apnéia obstrutiva do sono? teve como objetivo realizar a análise da VFC em trechos livres de apnéia durante o estágio 2 (S2) e sono REM em pacientes com SAOS leve, moderada e indivíduos sem SAOS. Foram incluídos no estudo 20 pacientes com SAOS leve e 20 com SAOS moderada e 18 pacientes sem SAOS. Todos os sujeitos foram submetidos à polissonografia (PSG) com gravação do eletrocardiograma (ECG). OS índices de VFC foram obtidos através da análise dos intervalos RR (i-RR) em trechos de 5 minutos livres de apnéia aplicando métodos lineares no domínio da frequência (LF, LF e razão LF/HF) e não lineares Poincarré Plot (SD1 e SD2), Recurrence Plot [(Mean line length (Lmean)], Recurrence rate (REC), Determinism (DET) e Entropia de Shannon (ShanEn)]. O grupo com SAOS moderada apresentou maiores valores de LF, LF/HF e DET, quando comparado ao grupo sem SAOS e menores valores de HF (que corresponde à atividade parassimpática) sugerindo uma hiperatividade simpática nesta população. Interessantemente, pacientes com SAOS leve falharam em demonstrar as diferenças esperadas através dos índices lineares entre os diferentes estágios de sono, como observado nos indivíduos sem SAOS, podendo representar um desbalanço autonômico precoce neste estágio de severidade de SAOS. Nossos dados sugerem que em pacientes com SAOS, ocorre um prejuízo crônico estágio dependente da modulação linear e não linear autonômica cardíaca. Interessantemente, este desbalanço pode ser identificado em fases mais leves da doença. O estudo II, intitulado: Variabilidade da frequência cardíaca e acoplamento cardiorrespiratório durante o sono de pacientes candidatos a cirurgia bariátrica teve como objetivos determinar se a gravidade da obesidade altera a regulação autonômica cardíaca e o acoplamento cardiorrespiratório durante o sono usando análise espectral da VFC e os sinais de variabilidade respiração (VR) em pacientes com indicação a cirurgia bariátrica (CB). 29 pacientes consecutivos em pré operatório de CB e 10 sujeitos (controles) submetidos a polissonografia. Parâmetros espectrais e de espectro cruzado (ACR) da VFC a VR foram analisados durante diferentes estágios do sono. As análises espectrais da VFC e da VR indicaram baixa regularidade da respiração durante o sono e baixa VFC nos pacientes obesos durante todos os estágios de sono quando comparados aos controles (p<0.05). Obesos graves e superobesos apresentam menores valores de razão LF/HF e potência LF durante o sono REM e altos valores de potência HF (p<0.05), enquanto obesos mórbidos apresentam menores valores de razão LF/HF e potência durante estágio 2 de sono (S2) quando comparados aos controles (p<0.05). Parâmetros do ACR demonstram que superobesos apresentaram menores % de potência coerente do tacograma com a respiração durante o estágio 3 de sono (S3) comparados os controles (p<0.05). Nossos dados sugerem que pacientes em pré operatório de CB apresentam VFC e VR alteradas durante todos os estágios de sono. Obesos graves, mórbidos e superobesos apresentam sincronia cardiorrespiratória alterada durante o sono e estas alterações estão correlacionadas com a severidade da obesidade e da SAOS. Nosso terceiro estudo intitulado: Contrastes da variabilidade da frequência cardíaca e do acoplamento cardiorrespiratório em jovens e idosos com SAOS , teve como objetivo contrastar a o impacto da SAOS entre jovens e idosos na VFC e no ACR. Além disso, visou estabelecer se a idade impacta diferentemente na VFC e no ACR durante as diferentes fases do sono, utilizando análise espectral da VFC e sinal de VR em jovens e idosos com e sem SAOS. As análises espectrais da VFC indicaram que a presença de SAOS impactou negativamente na VFC de jovens, com maiores valores de LF/HF (p<0,05) durante a vigília e nas diferentes fases do sono com menores valores de LF/HF durante S2 e sono REM (p<0,05). Observamos também que a idade afetou negativamente a VFC durante a vigília e durante o sono de idosos com maiores valores de LF/HF durante a vigília. Jovens com SAOS e idosos apresentaram menores valores de % de potência coerente do tacograma com a respiração apenas durante a vigília (p<0,05). A presença de SAOS impacta negativamente na VFC de jovens e de idosos, com redução na modulação autonômica durante a vigília e sono REM. A idade impacta negativamente na VFC durante a vigília e durante o sono de idosos.
