CLINICAL CHARACTERISTICS, EVOLUTION AND OUTCOMES OF PULMONARY ARTERIAL HYPERTENSION IN A REFERAL PERUVIAN HOSPITAL

Resumo: | Objectives. Describe the clinical characteristics, evolution, and treatment of patients with pulmonary arterial hypertension. Materials and methods: Descriptive study that included 47 patients with a primary diagnosis of pulmonary hypertension. Results: The average age was 48 years and 91% were female. The average follow-up was 39 months. The predominant types were associated with congenital heart disease (48.9%), associated with connective tissue disease (21.3%) and idiopathic (17%). Monotherapy, mainly Sildenafil was used in 78.7% and sequential combination therapy, mostly Sildenafil plus Bosentan in 21.3% of the cases. During follow-up, 70.2% had at least one hospitalization and five patients died, making a cumulative total mortality of 10.6%. Conclusions. Pulmonary arterial hypertension predominantly affects young and female people. The main etiology found was congenital heart disease. The most frequently used specific treatment was Sildenafil as monotherapy. The mortality found was lower than that reported in other registries. |

Describir las características epidemiológicas, clínicas, evolución y tratamiento de los pacientes con hipertensión arterial pulmonar. Materiales y métodos: estudio descriptivo que incluyó a 47 pacientes con diagnóstico primario de hipertensión pulmonar. Resultados: La edad promedio fue 48 años y 91% fueron de sexo femenino. El promedio de seguimiento fue 39 meses. Los tipos predominantes fueron; asociado a cardiopatía congénita (48,9%), asociado a enfermedad del tejido conectivo (21,3%) e idiopático (17%). Se usó monoterapia principalmente Sildenafilo en 78.7% y terapia combinada secuencial, en su gran mayoría Sildenafilo más Bosentan en 21.3% de los casos. Durante el seguimiento, el 70,2% tuvo por lo menos una hospitalización y cinco pacientes fallecieron, haciendo una mortalidad total acumulada de 10,6%. Conclusiones. Hipertensión arterial pulmonar afecta predominantemente a personas jóvenes y de sexo femenino. La principal etiología encontrada fue la cardiopatía congénita. El tratamiento específico más frecuentemente usado fue Sildenafilo como monoterapia.
