Impact of polypharmacy and drug use on stratifying the risk of falling patients in the hospital environment

Resumo: Falls can cause physical and / or psychological damage in hospitalized patients, in addition to increased length of stay and costs of hospitalization. Among the multiple risk factors associated with the occurrence of falls, the use of drugs that increase the risk and polypharmacy must be considered. Objectives: to estimate the magnitude of the effect of exposure of adult patients to polypharmacy and the use of Fall-Risk-Increasing Drugs on the risk of falls associated with the use of medications during hospitalization. Method: quantitative, cross-sectional study, carried out in a medium-sized general hospital, located in the city of Rio de Janeiro. Statistical analysis was performed using simple and multiple logistic regressions. Results: there was an association between the prescription of Fall-Risk-Increasing Drugs and the high risk of falling associated with the use of medications, with an OR of 13.53 95% CI (1.83 - 606.58). The occurrence of polypharmacy in the studied cohort was 81.4% (n = 57). Of the total number of patients who used polypharmacy (n = 57), 82.5% also used some Fall-Risk-Increasing Drugs. The number of patients with polypharmacy who used and did not use Fall-Risk-Increasing Drugs was statistically significant (p-value <0.05). Conclusion: the results of the study suggest that the use of Fall-Risk-Increasing Drugs is an important contributing factor to the increased risk and falls among patients using polypharmacy and therefore needs to be considered in the prevention protocols.

Entre los múltiples factores de riesgo asociados con la aparición de caídas, debe considerarse el uso de medicamentos que aumentan el riesgo y la polifarmacia. Objetivos: estimar la magnitud del efecto de la exposición de pacientes adultos a la polifarmacia y el uso de medicamentos que aumentan el riesgo de caídas sobre el riesgo de caídas asociadas con el uso de medicamentos durante la hospitalización. Método: estudio cuantitativo, transversal, realizado en un hospital general de tamaño medio, ubicado en la ciudad de Río de Janeiro. El análisis estadístico se realizó mediante regresiones logísticas simples y múltiples. Resultados: hubo una asociación entre la prescripción de medicamentos que aumentan el riesgo de caídas y el alto riesgo de caídas asociado con el uso de medicamentos, con un OR de 13.53 IC 95% (1.83 - 606.58). La aparición de polifarmacia en la cohorte estudiada fue del 81,4% (n = 57). Del número total de pacientes que usaron polifarmacia (n = 57), el 82.5% también usó algunos medicamentos que aumentan el riesgo de caídas. El número de pacientes con polifarmacia que usaron y no usaron medicamentos que aumentan el riesgo de caídas fue estadísticamente significativo (valor p <0.05). Conclusión: los resultados del estudio sugieren que el uso de medicamentos que aumentan el riesgo de caídas es un factor importante que contribuye al aumento del riesgo y cae entre los pacientes que usan polifarmacia y, por lo tanto, debe considerarse en los protocolos de prevención.Quedas podem gerar prejuízos físicos e/ou psicológicos nos pacientes hospitalizados, além de aumento do tempo de permanência e custos da internação. Dentre os múltiplos fatores de risco associados à ocorrência de quedas, o uso de medicamentos que potencializam o risco e a polifarmácia devem ser considerados. Objetivos:  estimar a magnitude do efeito da exposição de pacientes adultos à polifarmácia e ao uso de Fall-Risk-Increasing Drugs sobre o risco de queda associado ao uso de medicamentos durante a internação hospitalar. Métodos: estudo quantitativo, transversal, realizado em um hospital geral de médio porte, localizado na cidade do Rio de Janeiro. A análise estatística foi realizada a partir de regressões logísticas simples e múltiplas. Resultados: observou-se associação entre a prescrição de Fall-Risk-Increasing Drugs e o alto risco de queda associado ao uso de medicamentos, com  OR de 13,53 IC 95% (1,83 – 606,58). A ocorrência de polifarmácia na coorte estudada foi de 81,4% (n=57). Do total de pacientes que fizeram uso de polifarmácia (n=57), 82,5% também fizeram uso de algum Fall-Risk-Increasing Drugs. O número de pacientes com polifarmácia que usaram e não usaram Fall-Risk-Increasing Drugs foi significativo estatisticamente (p-valor < 0,05).
