PHC gaps for people with stigmatized manifestations of functional diversity (with disabilities): A participatory and integrative review

Resumo: This paper maps the gaps in Primary Health Care perceived and experienced by disabled persons to subsidize health care network, starting from collective appropriation of academic and technical-institutional production. The methodology consisted in integrative and participative literature review, selecting papers from 2008 to 2021, in the following bases: Lilacs, Embase, PubMed, Scopus and Web of Science and Google Scholar databases. The chosen studies were discussed over a year with researchers, in simultaneous stages, health workers and managers, disabled people and social movement leaders. The theoretical framework is inspired by the emancipatory critical theory and considers the centrality of intersectionality for the understanding of health inequities mobilized by disability and for the proposition of public policies. It was identified that the observed progress in the proposal of a comprehensive primary care does not fully materialize for disabled persons, who face, when accessing Primary Health Care services, limiting or preventing barriers to the bond formation, the resignification of this group, the resoluteness of the comprehensive and coordinated health, from prevention and health promotion, fragmenting care and impacting in the quality of care offered with disproportionate effects for this population. Overcoming these gaps is a fundamental challenge for the policy of attention to disabled persons.

La metodología utilizada fue la revisión bibliográfica integradora y participativa, siendo seleccionados textos de 2008 a 2021, en las bases de datos Lilacs, Embase, Pubmed, Scopus y Web of Science y Google Scholar. Los estudios seleccionados se debatieron con investigadores, trabajadores y gestores sanitarios, PcD y líderes de movimientos sociales, en etapas simultáneas a lo largo de un año. El marco teórico se inspira en la teoría crítica emancipadora y considera la centralidad de la interseccionalidad para la comprensión de las desigualdades en salud movilizadas por la discapacidad y la proposición de políticas públicas. Identificou-se que o progresso na proposta de uma atenção primária abrangente não se materializa plenamente para as pessoas com deficiência que, ao acesso aos serviços de APS, enfrentam barreiras à formação de vínculo, à ressignificação desse grupo populacional, à resolutividade da saúde integral e coordenada, desde a prevenção e a promoção em saúde, marcando a fragmentação do atendimento e a qualidade da atenção ofertida com efeitos desproporcionais para essa população. Superar estas lagunas es un reto fundamental de la política de atención a las PcD.Esse artigo mapeia as lacunas sanitárias na APS vividas pelas pessoas com deficiência (PcD), subsidiando a qualificação da rede de atenção à saúde a partir de esforço coletivo para apropriação da produção acadêmica e técnico-institucional. A metodologia utilizada foi a revisão integrativa e participativa da literatura, sendo selecionados textos de 2008 a 2021, nas bases Lilacs, Embase, Pubmed, Scopus e Web of Science e Google Scholar. Os estudos escolhidos foram debatidos com pesquisadores, trabalhadores e gestores da saúde, PcD e lideranças dos movimentos sociais, em etapas simultâneas ao longo de um ano. O referencial teórico se inspira na teoria crítica emancipatória e considera a centralidade da interseccionalidade para entendimento das iniquidades de saúde mobilizadas pela deficiência e proposição de políticas públicas. Identificou-se que o progresso na proposta de uma atenção primária abrangente não se materializa plenamente para as pessoas com deficiência que, ao acessar os serviços de APS, encaram barreiras à formação de vínculo, à ressignificação desse grupo populacional, à resolutividade da saúde integral e coordenada, desde a prevenção e a promoção em saúde, marcando a fragmentação do atendimento  e a qualidade da atenção ofertada com efeitos desproporcionais para essa população.
