EVALUATION OF THE COST, EFFECTIVENESS AND SATISFACTION OF ACTORS IN A HOME BLOOD GLUCOSE MONITORING: PROGRAMA DE MONITORIZAÇÃO GLICÊMICA DOMICILIAR

Resumo: Introduction: Glycemic monitoring is a strategy that promotes education for supported self-care and the effective participation of individuals who are susceptible or have diabetes mellitus. Objective: To evaluate the cost, effectiveness and satisfaction of actors in a home glycemic monitoring program. Method: Sequential mixed method study, from 2019 to 2021. In phases 1 and 2, quantitative data, a micro-funding study was carried out. The results of the program were analyzed through document analysis, identifying the absolute and relative frequency of dependent and independent variables, with mean and standard deviation being calculated. In phase 3, qualitative data, the strengths and weaknesses of the program were evaluated. In the analysis, the data quantitative and qualitative were connected. Results: The monthly cost of the patient who received 50 and 100 units of supplies was R$89.89 (U$17.29) and R$147.99 (U$28.46), respectively. A total of 659 patients were evaluated, with a reduction of 0.84% in glycated hemoglobin and a reduction of 23.29 mg/dl in fasting glucose. As potentialities, stands out the free availability of inputs stands out; aid in therapeutic management; development of skills for self-care. As weakness, stands out bureaucracy and rigor in the control and distribution of inputs; difficulty understanding and correctly adhering to blood glucose monitoring; lack of expanded multidisciplinary team in care; and the need for technological improvements for analysis and monitoring of results. Conclusion: The costs related to the program can be a positive investment, demonstrated by the satisfaction of the actors, which presented a balance between strengths and weaknesses.

Objetivo: Evaluar el costo, la efectividad y la satisfacción de los actores de un programa de monitoreo glucémico domiciliario. Método: Estudio de método mixto secuencial, de 2019 a 2021. En las etapas 1 y 2, datos cuantitativos, se realizó un estudio de microfinanciamiento y se analizaron los resultados del programa a través del análisis de documentos, identificando la frecuencia absoluta y relativa de variables dependientes e independientes, calculándose la media y la desviación estándar. En el paso 3, datos cualitativos, se evaluaron las fortalezas y debilidades del programa. En el análisis, se conectaron los datos cuantitativos e cualitativos. Resultados: El costo mensual del paciente que recibió 50 y 100 unidades de insumos fue de R$ 89,89 (U$ 17,29) y R$ 147,99 (U$ 28,46), respectivamente. Se evaluaron un total de 659 pacientes, con una reducción de 0,84% en la hemoglobina glicosilada y una reducción de 23,29 mg/dl en la glucosa en ayunas. Objetivo: Avaliar o custo, a eficácia e a satisfação dos atores de programa de monitorização glicêmica domiciliar. Método: Estudo de método misto sequencial, no período de 2019 a 2021. Nas etapas 1 e 2 (“QUAN”), realizou-se estudo de microcusteio e foram analisados os resultados do programa por análise documental, identificando a frequência absoluta e relativa das variáveis dependentes e independentes, sendo calculados a média e o desvio padrão. Na etapa 3 (“qual”), foram avaliadas as potencialidades e fragilidades do programa. Na análise foram conectados os dados “QUAN” e “qual”. Resultados: O custo mensal do paciente que recebeu 50 e 100 unidades de insumos foi de R$89,89 (U$17,29) e de R$147,99 (U$28,46), respectivamente. Foram avaliados 659 pacientes, com redução de 0,84% na hemoglobina glicada e redução de 23,29 mg/dl na glicemia de jejum. Como potencialidades destaca-se a disponibilização gratuita de insumos; auxílio no manejo terapêutico; desenvolvimento da habilidade para o autocuidado; e estabelecimento de diretriz/protocolo para distribuição dos insumos. E, como fragilidades: burocracia e rigor no controle e distribuição dos insumos; dificuldade de compreensão e adesão correta ao monitoramento da glicemia; falta de equipe multiprofissional ampliada no atendimento; e necessidade de melhorias tecnológicas para análise e monitoramento dos resultados.
