DIFFICULTIES OF THE ELDERLY IN ADHERING TO TREATMENT FOR CHRONIC NON-COMMUNICABLE DISEASES

Resumo: | Introduction: Ageing is a biological and universal phenomenon, as it involves physical, social, psychological and spiritual changes. Low adherence in the elderly population has been observed, mainly due to the diagnosis of more than one type of disease, which requires various medications and care. Objective: To review the literature in order to identify the difficulties faced by elderly people with chronic non-communicable diseases in adhering to therapeutic treatment. Methods: Integrative literature review, based on the analysis of 18 articles, carried out between January and March 2023. We included full-text articles on people over the age of 60, in Portuguese, English and Spanish, published in the last 10 years (2014-2023), available for free download and which answered the research question. Monographs, theses, dissertations, repeated and review articles, on the other hand, were excluded from the sample. Results: The main difficulties faced by the elderly in adhering to therapeutic treatment are related to various factors, such as medication side effects, forgetfulness, advanced age, multimorbidities, schooling, failure in communication between patient and professional and high medication costs. Discussion: Recognizing difficulties in medication adherence contributes to better disease management and, consequently, to the quality of life of elderly people with chronic non-communicable diseases. Conclusion: The elderly need specialized attention to understand the disease and the importance of therapeutic treatment. |

Se ha observado una baja adherencia en la población anciana, principalmente debido al diagnóstico de más de un tipo de enfermedad, que requiere diversos medicamentos y cuidados. Objetivo: Revisar la literatura para identificar las dificultades que enfrentan los ancianos con enfermedades crónicas no transmisibles en la adherencia al tratamiento terapéutico. Métodos: Revisión bibliográfica integradora, basada en el análisis de 18 artículos, realizada entre enero y marzo de 2023.Se incluyeron artículos a texto completo sobre personas mayores de 60 años, en portugués, inglés y español, publicados en los últimos 10 años (2014-2023), disponibles para descarga gratuita y que respondieran a la pregunta de investigación. Monografías, tesis, disertaciones, artículos repetidos y de revisión, por otro lado, fueron excluidos de la muestra. Resultados: Las principales dificultades de las personas mayores para adherirse al tratamiento terapéutico están relacionadas con diversos factores, como los efectos secundarios de la medicación, el olvido, la edad avanzada, las multimorbilidades, la escolarización, el fallo en la comunicación entre paciente y profesional y el elevado coste de la medicación. Discusión: Reconocer las dificultades en la adherencia a la medicación contribuye a un mejor manejo de la enfermedad y, en consecuencia, a la calidad de vida de las personas mayores con enfermedades crónicas no transmisibles. É constatada a baixa adesão na população idosa, principalmente, em virtude do diagnóstico de mais de um tipo de doença, o que demanda várias medicações e cuidados. Objetivo: Revisar a literatura para identificar as dificuldades do idoso com doença crônica não transmissível na adesão ao tratamento terapêutico. Métodos: Revisão integrativa da literatura, baseada na análise de 18 artigos, realizada entre janeiro e março de 2023. Foram incluídos artigos com texto completo, desenvolvidos com pessoas com idade acima de 60 anos, nos idiomas português, inglês e espanhol, publicados nos últimos 10 anos (2014-2023), disponíveis para download gratuito e que respondessem à pergunta de pesquisa. Já as monografias, teses, dissertações, artigos repetidos e de revisão foram excluídos da amostra. Resultados: As principais dificuldades dos idosos na adesão ao tratamento terapêutico estão relacionadas aos diversos fatores, como efeitos colaterais da medicação, esquecimento, idade avançada, multimorbidades, escolaridade, falha na comunicação entre paciente e profissional e altos custos das medicações. Discussão: O reconhecimento de dificuldades na adesão medicamentosa contribui para o melhor manejo da doença e, consequentemente, na qualidade de vida das pessoas idosas com doenças crônicas não transmissíveis.
