LEPROSY IN CHILDREN UNDER FIFTEEN YEARS OLD IN AMAZONAS: CLINICAL AND EPIDEMIOLOGICAL ASPECTS AND SPATIAL DISTRIBUTION

Resumo: | ABSTRACT Introduction: Leprosy is an infectious, transmissible, and chronic disease that can cause severe and permanent physical disabilities. Children under 15 years old affected by this illness, without early diagnosis or proper treatment, may acquire sequelae that will affect their adult life. Objective: To identify the epidemiological, clinical aspects, and spatial distribution of reported leprosy cases in individuals under 15 years old in the state of Amazonas between 2002 and 2021. Method: This is an epidemiological, ecological, and retrospective research, using secondary data from the Notifiable Diseases Information System available in the database of the Unified Health System's Informatics Department. The detection rate of cases per year was calculated, and descriptive analyses were performed using Microsoft Excel, and the Quantum GIS (QGIS) software, version 2.18.6, for map analysis and construction. Results: A total of 1,471 cases were reported, with a prevalence of: female sex (51.3%), age between 10 and 14 years (70.2%), mixed race (67.9%), residence in rural areas (62.5%), paucibacillary operational classification (63.6%), tuberculoid clinical form (32.9%), grade 0 physical disability in the notification (80.6%) and in the cure (63.3%). Over the years, a decrease in the detection rate was observed. Conclusion: Considering the high endemicity of leprosy in children under 15 years old in the Amazonas region, the importance of developing more effective disease control and dissemination strategies, as well as prevention of disabilities, particularly in children, is emphasized. |

Los niños menores de 15 años que padecen esta enfermedad y no reciben un diagnóstico temprano o un tratamiento adecuado pueden adquirir secuelas que afectarán su vida adulta. Objetivo: Identificar los aspectos epidemiológicos, clínicos y la distribución espacial de los casos notificados de lepra en menores de 15 años en el Estado de Amazonas entre 2002 y 2021. Método: Se trata de una investigación epidemiológica, ecológica y retrospectiva, con datos secundarios del Sistema de Información de Agravios de Notificación disponible en la base del Departamento de Informática del Sistema Único de Salud. Se calculó la tasa de detección de casos por año, además de análisis descriptivos mediante Microsoft Excel y el programa Quantum GIS (QGIS), versión 2.18.6, para análisis y construcción de mapas. Resultados: Se notificaron 1.471 casos, con una prevalencia de: sexo femenino (51,3%), edad entre 10 y 14 años (70,2%), raza parda (67,9%), residencia en el interior del estado (62,5%), clasificación operacional paucibacilar (63,6%), forma clínica tuberculoide (32,9%), grado de discapacidad física 0 en la notificación (80,6%) y en la cura (63,3%). A lo largo de los años, se observó una disminución en la tasa de detección. Conclusión: Dada la alta endemicidad de la lepra en menores de 15 años en Amazonas, se destaca la importancia de desarrollar estrategias más eficientes para controlar la enfermedad y su propagación, así como la prevención de discapacidades, especialmente en la población infantil.RESUMO Introdução: A hanseníase é uma doença infecciosa, transmissível e de caráter crônico que pode causar incapacidades físicas severas e permanentes. Pessoas com idade abaixo de 15 anos que são acometidas por essa enfermidade e não recebem diagnóstico precoce ou tratamento adequado, podem adquirir sequelas que afetarão sua vida adulta. Objetivo: Identificar os aspectos epidemiológicos, clínicos e a distribuição espacial dos casos notificados de hanseníase em menores de 15 anos no Estado do Amazonas entre 2002 e 2021. Método: Trata-se de uma pesquisa epidemiológica, ecológica e retrospectiva, com dados secundários do Sistema de Informação de Agravos de Notificação disponível na base do Departamento de Informática do Sistema Único de Saúde. Foi realizado o cálculo da taxa de detecção dos casos por ano, além de análises descritivas pelo Microsoft Excel, e, o programa Quantum GIS (QGIS), versão 2.18.6, para análise e construção dos mapas.
