PROFILE OF MEDICATION ERRORS BY ENTERAL TUBE IN INTENSIVE CARE: : AN OBSERVATIONAL STUDY

Resumo: Objective: to analyze the errors that occur in the preparation of medications by enteral tubes in intensive care. Method: observational study, with cross-sectional design, guided by the recommendations of the Strengthening the Reporting of Observational Studies in Epidemiology (STROBE), carried out in four intensive care units of a private hospital in the city of Rio de Janeiro. To evaluate the preparation of drugs in solid pharmaceutical form, the variables were: crushing and dilution. For liquid drugs only the dilution variable. The unit of measurement used for the sample calculation was the prepared dose of the drug, and 720 doses of drugs prescribed by probes were analyzed. We observed 80 nursing technicians at the time of preparation of the medications, ensuring that the professional was observed in the management of at least nine doses of medications.  Results: It was identified error in the preparation of 480 doses (66.66%). The coated tablets and hard gelatin capsules were improperly crushed in 100% of the doses analyzed and 54% of the simple tablets had insufficient grinding. Insufficient dilution occurred in 100% of the syrup doses. Insufficient grinding for single tablets occurred in 310 doses (54%). Insufficient dilution occurred in 100% of the syrup doses. The prevalent error was that of grinding for solid drugs. Conclusions: the preparation of drugs presented a high frequency of errors, which can lead to losses in the bioavailability of the drug and compromise the good practices of enteral tube drug therapy.

Para evaluar la preparación de fármacos en forma farmacéutica sólida, las variables fueron: trituración y dilución. Para medicamentos líquidos solo la variable de dilución. La unidad de medida utilizada para el cálculo de la muestra fue la dosis preparada del fármaco, y se analizaron 720 dosis de fármacos prescritos por sondas.   Se observaron 80 técnicos de enfermería en el momento de la preparación de los medicamentos, asegurando que el profesional fuera observado en el manejo de al menos nueve dosis de medicamentos.  Resultados: fue identificado error en la preparación de 480 dosis (66,66%). Los comprimidos recubiertos y las cápsulas de gelatina dura se trituraron incorrectamente en el 100% de las dosis analizadas y el 54% de los comprimidos simples tuvieron una molienda insuficiente. La dilución insuficiente ocurrió en el 100% de las dosis de jarabe. La molienda insuficiente para comprimidos individuales ocurrió en 310 dosis (54%). La dilución insuficiente ocurrió en el 100% de las dosis de jarabe. El error prevalente fue el de moler para obtener medicamentos sólidos. Conclusiones: la preparación de los fármacos presentó una alta frecuencia de errores, lo que puede conducir a pérdidas en la biodisponibilidad del fármaco y comprometer las buenas prácticas de la terapia farmacológica por tubo enteral.Objetivo: analisar os erros que ocorrem no preparo de medicamentos por sondas enterais em terapia intensiva. Para avaliar o preparo de medicamentos na forma farmacêutica sólida as variáveis foram: trituração e diluição. Para os medicamentos líquidos somente a variável diluição. A unidade de medida utilizada para o cálculo amostral foi a dose preparada do medicamento, sendo analisadas 720 doses de medicamentos prescritos por sondas.  Foram observados 80 técnicos de enfermagem no momento de preparo dos medicamentos garantindo-se que o profissional fosse observado no manejo de no mínimo nove doses de medicamentos. Resultados: Foi identificado erro no preparo de 480 doses (66,66%). Os comprimidos revestidos e cápsulas de gelatina dura foram triturados indevidamente em 100% das doses analisadas e 54% dos comprimidos simples tiveram tritura insuficiente. A diluição insuficiente ocorreu em 100% das doses de xaropes. A tritura insuficiente para comprimidos simples ocorreu em 310 doses (54%). A diluição insuficiente ocorreu em 100% das doses de xaropes. O erro prevalente foi o de tritura para os medicamentos sólidos.
