IS PRESSURE INJURY RESULTING FROM SURGICAL POSITIONING ALWAYS PREVENTABLE? REFLECTING WITH A CLINICAL CASE

Resumo: Objective: To reflect, in the light of Critical Thinking, from the report of a clinical case of pressure injuries resulting from surgical positioning, about interventions for its prevention and about the limits of avoidability of the adverse event. Method: Study of a case of pressure injuries resulting from surgical positioning that occurred in a university hospital in Northeast Brazil, in March 2021. Guiding questions were used to stimulate Critical Thinking skills. Results: The patient had 22 points on the Injury Risk Assessment Scale resulting from Surgical Positioning, corresponding to high risk. After 16:50 of surgery, he presented five pressure injuries. Conclusion: It was evidenced that, with prevention failures, the confluence of avoidable and non-preventable injury risk factors was decisive for the development of injuries. It is not possible to assure that injuries could not have been avoided when there are failures in prevention.

Método: Estudio de un caso de lesiones por presión resultantes de posicionamiento quirúrgico ocurrido en un hospital universitario en el Nordeste de Brasil, en marzo de 2021. Se utilizaron preguntas orientadoras para estimular las habilidades de Pensamiento Crítico. Resultados: El paciente presentó 22 puntos en la Escala de Valoración de Riesgo de Lesión resultante del Posicionamiento Quirúrgico, correspondientes a alto riesgo. Después de las 16:50 de la cirugía, presentó cinco lesiones por presión. Conclusión: Se evidenció que, con las fallas de prevención, la confluencia de factores de riesgo de lesiones evitables y no evitables fueron determinantes para el desarrollo de lesiones. No es posible asegurar que las lesiones no se podrían haber evitado cuando hay fallas en la prevención.Objetivo: Refletir, à luz do Pensamento Crítico, a partir do relato de um caso clínico de lesões por pressão decorrentes do posicionamento cirúrgico, acerca das intervenções para sua prevenção e sobre os limites de evitabilidade do evento adverso. Método: Estudo de um caso de lesões por pressão decorrentes do posicionamento cirúrgico ocorrido em um hospital universitário do Nordeste do Brasil, em março de 2021. Foram utilizadas questões norteadoras para estimular as habilidades do Pensamento Crítico. Resultados: O paciente apresentava 22 pontos na Escala de Avaliação de Risco de Lesões decorrentes do Posicionamento Cirúrgico, correspondente a alto risco. Após 16h50 de cirurgia, apresentou cinco lesões por pressão. Conclusão: Evidenciou-se que, com as falhas na prevenção, a confluência de fatores de risco de lesão evitáveis e não-evitáveis foram determinantes para o desenvolvimento das lesões.
