To identify incidents that occurred with PHC users in the city of Escada-PE. Materials and Methods: A cross-sectional, descriptive, quantitative and qualitative study. The data were analyzed with the support of the software EpiInfo version 7.2 and the narratives were analyzed by the Discourse of the Subject Collective with DSCSoft2.0 support. Results: It can be verified that 40% of the incidents are related to a particular patient, where the majority of the patient is female 78.94%, presenting the average age of 41.41 years, being most of the error indicated as moderate 10.00% mainly caused by the office 20.00%. It is possible to list five discourses in five thematic categories: Failure to use material ineffective / insufficient, Failure in care, Administrative failure, Failure with exploratory examination and Failure of technical competence. Conclusions: The studies show that it requires the joint work between the management and all the professionals of the Basic Health Unit, in order to provide decent work environment and qualified care .
