Vivência de cuidadores familiares de usuários de álcool que necessitaram de internação em terapia intensiva

Resumo: | Hospitalizations of patients with acute or chronic complications from alcohol abuse are quite common. However, hospitalization in intensive care units (ICU) is a critical and difficult experience for users of this type of drugs and for their families, because it means that the problem of alcohol abuse took a larger dimension in their lives, with the possibility of sequels and death as the outcome. An integrative review was conducted prior to the field work, using the descriptors "family" and "alcohol user" combined, showing 13 publications approaching the family relationship of alcohol users. It was a small number considering the importance of the topic, fundamental to support clinical practice and nursing care. Given these considerations, the present study aimed to understand the experience of family caregivers of patients who were alcohol users admitted to intensive care. This is an exploratory-descriptive study, carried out with patients with medical diagnoses associated with chronic or acute alcohol abuse, who were hospitalized in the adult intensive care unit of an education hospital in the Northwest of Paraná state, from January to December 2011. Data collection was conducted through semi-structured interviews with 10 family caregivers of patients. The quantitative data, for the characterization of the alcohol users, family caregivers and families were described in absolute frequency, and for the qualitative data analysis, it was used the Thematic Content Analysis. The quotidian homecare provided to alcohol users was gathered into three units of analysis ? Care given before and after hospitalization in the intensive care unit; Care given to a family member who is an alcohol abuser; Family support as a facilitator in the caregiving process. As for the feelings expressed by the family caregivers, two units of analysis were defined: Admission of alcohol users into the ICU - the pain and suffering of their family caregivers; The religiosity as a way coping the suffering. Regarding the alcohol users, most of them were male, 45.1 years average, divorced, with little schooling and unemployed. All were hospitalized due to chronic effects of alcohol intoxication, the average time of hospitalization was 40.3 days and three of them died. Five families were making less than three minimum wages of income and alcohol addiction by other members of their families was present in seven of them. The family caregivers were mostly female, usually sisters of the patients, and 46.7 years average. Seven were married and possessed low level of education. Homecare activities included surveillance and guidance for abstinence before ICU admission; care that aimed to either repair or treat the disease, immediately after discharge; surveillance activities to prevent relapse, and patient safety at the time of the interview. Caring and living with individuals addicted to alcohol was a difficult situation because their caregivers were often exposed to violence. The involvement of other family members in the caring process was an important facilitating factor for the caregiver to provide care, and despite the daily difficulties, the affection for the family outweighed the overload generated by the caregiving process. The main feelings reported by the family caregivers, regarding the admission of alcohol users in ICU, were suffering, sorrow, despair and guilt. Religiosity appeared as a way of coping with suffering. No entanto, a internação em Unidades de Terapia Intensiva (UTI) representa um momento crítico e de difícil experiência para os usuários dessa droga e seus familiares, pois significa que o problema do abuso do álcool tomou dimensões maiores em suas vidas, com possibilidade de sequelas e a morte como desfecho. Em revisão integrativa, realizada previamente ao trabalho de campo, a partir dos descritores "família" e "usuário de álcool" combinados, foram encontradas 13 publicações que contemplavam as relações familiares de usuários de álcool, um número pequeno diante da importância do tema, fundamental para subsidiar a prática clínica e o cuidado de enfermagem. Diante dessas considerações, o presente estudo objetivou compreender a vivência de cuidadores familiares de pacientes usuários de álcool que estiveram internados em terapia intensiva. Estudo exploratório-descritivo, com abordagem de pacientes com diagnósticos médicos associados ao uso agudo ou crônico de álcool que foram internados em Unidade de Terapia Intensiva adulto de um hospital ensino da região Noroeste do Paraná no período de janeiro a dezembro de 2011. A coleta de dados foi realizada por meio de entrevista semiestruturada com dez cuidadores familiares dos pacientes. Os dados quantitativos, para caracterização dos usuários de álcool, cuidadores familiares e famílias foram descritos em frequência absoluta, e para análise dos dados qualitativos, foi utilizada a Análise de Conteúdo Temática. O cotidiano do cuidado domiciliar a usuários de álcool foi reunido em três unidades de análise - Atividades do cuidar antes e após a internação em terapia intensiva; O cuidar de um membro familiar usuário abusivo de álcool; O apoio familiar como facilitador no processo de cuidar. Quanto aos sentimentos expressados pelos cuidadores familiares, demarcaram-se duas unidades de análise - A internação dos usuários de álcool em UTI: a dor e o sofrimento de seus cuidadores familiares; A religiosidade como forma de enfrentamento ao sofrimento. Em relação aos usuários de álcool, a maioria era do sexo masculino, com média de idade de 45,1 anos, divorciados, de baixa escolaridade e fora do mercado de trabalho. Todos foram internados por efeitos da intoxicação alcoólica crônica, a média da internação hospitalar foi de 40,3 dias e ocorreram três óbitos. Cinco famílias apresentavam renda inferior a três salários mínimos e a dependência química do álcool por outros membros, esteve presente em sete delas. Entre os cuidadores familiares, a média de idade foi de 46,7 anos, sendo a maioria do sexo feminino e irmãs dos pacientes. Sete eram casados e apresentavam baixa escolaridade. Os cuidados domiciliares incluíram atividades de vigilância e orientação para a abstinência, antes da internação na UTI; cuidados que visaram à reparação ou ao tratamento da doença, imediatamente após a alta hospitalar; e atividades de vigilância, para evitar recaídas, e de segurança do paciente, no momento da entrevista. Cuidar e co-habitar com usuários dependentes de álcool, era uma situação difícil, pois seus cuidadores muitas vezes estiveram expostos à violências. A colaboração dos demais familiares no processo de cuidar era importante fator facilitador ao cuidador para a prestação de cuidados, e apesar das dificuldades cotidianas, o afeto pelo familiar se sobrepunha à sobrecarga gerada pelo processo de cuidar. Os principais sentimentos relatados pelos cuidadores familiares em relação à internação dos usuários de álcool em UTI foram sofrimento, tristeza, desespero e culpa. A religiosidade apareceu como forma de enfrentamento ao sofrimento. A internação dos usuários de álcool em UTI mostrou-se como um evento difícil de ser vivenciado pela família e principalmente pelos seus cuidadores familiares que, não obstante a condição de conviver no ambiente de alcoolismo depara-se com o evento da internação nesta unidade. Embora o caráter de alta tecnologia e complexidade dessas unidades, é importante conhecer o contexto familiar e social do cuidador dos pacientes pós-alta hospitalar, pois o entendimento da realidade no qual ele está inserido, possibilita identificar as reais necessidades de assistência ao mesmo, a quem ele cuida e à família, que norteará as orientações no momento da saída da unidade.
