Influência do nível de atividade física no metabolismo ósseo em indivíduos submetidos à cirurgia bariátrica

Resumo: | Bariatric surgery (CB) is a method of treating obesity that has been featured in recent years mainly by two factors: the exponential increase in cases of obesity and extreme obesity, and has been shown to be good possibility for the treatment of obesity especially for patients who require large weight losses. The main advantages of this method is the superior weight loss, maintenance of this situation in medium and long term, along with the resolution of comorbidities. On the other hand, there is the need of monitoring and evaluations to minimize the risk of bone loss, and the need to adopt an active lifestyle to prevent weight regain. However, there are few studies that verify the physical activity levels in individuals undergoing bariatric surgery. Accordingly, the present study aims to analyze the level of physical activity, along with indicators of bone metabolism in patients submitted to bariatric surgery. The sample consisted in 89 subjects who underwent bariatric surgery, categorized according to the time of surgery. In addition to the individuals operated, we selected a sample of 29 individuals in equivalent age, sex, weight and height, specifically evaluated for comparison with the group operated. We conducted tests of bone mineral density by DXA and blood parameters related to bone metabolism. All the assessments were performed at the Núcleo de Estudos Multiprofissional da Obesidade (NEMO), and the equipment required for the measures are part of the laboratory. A significant difference was found in the variables PTH of group G3 in relation to G1 and G2 and serum calcium of G3 group with the G1. The variable urinary calcium of groups G1, G2 and G3 showed percentage changes below normal levels of 12.5%, 23.9% and 11.1%, respectively. No associations were found between diagnosis of variables and physical activity level of the total sample. Regarding the percentage of results, both groups (active and sedentary) showed similar results, except for the variable blood glucose and total cholesterol, in which were found a higher prevalence of abnormal values in the sedentary group. The lowest BMD values were found in the operated group, showing significant differences in total body, femoral neck and total femur. Associations were found between surgery and diagnosis of bone mineral density of the total body and total femur, indicating an odds ratio of 2.33 (1.12 to 4.84) and 2.38 (1.15 to 4.93 ), respectively. Thus, it is necessary to develop more research on the influence of physical activity and long-term monitoring of these and other patients in order to provide useful information on bone changes from bariatric surgery. |

Uma das principais vantagens desse método é a acentuada perda de peso, a manutenção desse quadro em médio e longo prazo, juntamente com a resolução de comorbidades. Por outro lado, existe a necessidade de monitoramento e avaliações para minimizar os riscos de perda óssea, além da necessidade de adoção de um estilo de vida ativo, para evitar a recuperação do peso. Entretanto, são escassos os estudos que verificam os níveis de atividade física em indivíduos submetidos à cirurgia bariátrica. Nesse sentido, o presente estudo tem como objetivo analisar o nível de atividade física, juntamente com indicadores do metabolismo ósseo entre pacientes submetidos à Cirurgia Bariátrica. Foram avaliados 89 indivíduos que realizaram a cirurgia bariátrica, categorizados de acordo com o tempo de cirurgia. Além dos indivíduos operados, foi selecionada uma amostra de 29 indivíduos com equivalência em idade, sexo, peso e estatura, avaliada especificamente para fins de comparação com o grupo operado. Foram conduzidos exames das densidades minerais ósseas pela Absortometria Radiológica por dupla emissão de raios X (DXA) e parâmetros sanguíneos relacionados ao metabolismo ósseo. Todas as avaliações foram realizadas no Núcleo de Estudos Multiprofissional da Obesidade (NEMO), sendo que os equipamentos necessários para as medidas fazem parte do referido laboratório. Foi encontrada diferença significativa nas variáveis Hormônio Paratireóideo (PTH) do grupo G3 em relação aos grupos G1 e G2 e cálcio sérico do grupo G3 com o grupo G1. A variável cálcio urinário dos grupos G1, G2 e G3 apresentou percentuais de alterações abaixo dos níveis normais de 12,5%, 23,9% e 11,1%, respectivamente. Não foram encontradas associações entre o diagnóstico das variáveis e o nível de atividade física da amostra total. Em relação aos percentuais de alterações, os dois grupos (ativos e sedentários) apresentaram resultados similares, exceto para as variáveis glicemia e colesterol total, em que foram encontrados maiores prevalências de valores alterados no grupo sedentário. Os menores valores de Densidades Minerais Ósseas (DMO) foram encontrados no grupo operado, apresentando diferenças significativas no corpo total, fêmur e fêmur total. Foram encontradas associações entre a cirurgia e o diagnostico da densidade mineral óssea do corpo total e do fêmur total, indicando uma razão de chance de 2,33 (1,12-4,84) e 2,38 (1,15-4,93) respectivamente. Quando comparados com o grupo equivalente não operado, os indivíduos operados apresentaram uma chance aumentada de apresentarem alterações na densidade mineral óssea em mais de uma região avaliada.
