Desenvolvimento do gel Malva sylvestris L. e a avaliação clínica na resposta inflamatória gengival e no biofilome dental

Resumo: | In this study, a phytochemical screening and the in vitro antimicrobial and antioxidant properties of crude hydroalcoholic extract of Malva sylvestris L. (EHBM) was performed for a M. sylvestris gel formulation. Antiplaque and antigingivitis were also carried out using a partial-mouth, experimental gingivitis model in humans. Thirty subjects were chosen to participate in this crossover double-blind study, three 15-days experimental periods with a 10-days interval between each treatment. Patients were submitted to 3 treatments: Treatment 1 – placebo gel, Treatment 2 – 10% M.sylvestris gel; Treatment 3 – 2% chlorhexidine gel (positive control). The clinical parameters plaque index, modified marginal bleeding index and gingival fluid measurement were evaluated. The teeth: 15, 16, 35 and 36 were analyzed. EHBM showed the presence of mucilage (primary metabolite), steroids and/or triterpenes, coumarins, anthraquinones, flavonoids and tannins (secondary metabolites). The antioxidant activity mean was 5.75% and 26.30% compared to 100% of the total antioxidant activity attributed to ascorbic acid, respectively for EHBM and 10% M.sylvestris gel. No concentration of EHBM revealed antimicrobial effect in both agar disk and agar well diffusion tests. In the clinical study, there was no statistical difference for clinical parameters and gingival fluid measurement considering the different treatments. The primary and secondary metabolites of M. sylvestris suggest its anti-inflammatory effect. The EHBM has a reduced antioxidant activity. No influence on supragingival plaque is observed between 10% M. sylvestris gel and placebo. For gingival inflammation, no influence is verified for 10% M. sylvestris gel compared to placebo and chlorhexidine. |

For gingival inflammation, no influence is verified for 10% M. sylvestris gel compared to placebo and chlorhexidine.Os objetivos dessa pesquisa foram realizar a prospecção fitoquímica do extrato hidroalcoólico bruto (EHBM) de Malva sylvestris L., determinar in vitro a atividade antimicrobiana e propriedade antioxidante e in vivo a ação antiplaca e antigengivite do EHBM utilizando um modelo de gengivite experimental parcial em humanos. Trinta indivíduos foram selecionados para participar deste estudo cruzado duplocego, no qual foram estabelecidos três períodos experimentais de 15 dias cada um com um intervalo de 10 dias entre cada tratamento. Os pacientes foram submetidos a 3 tratamentos: Tratamento 1- Gel placebo, Tratamento 2- Gel de M. sylvestris a 10%; Tratamento 3- Gel de clorexidina a 2% (controle positivo). Os parâmetros clínicos avaliados foram o índice de placa, índice de sangramento marginal modificado e o volume de fluido gengival. Os dentes analisados foram: 15, 16, 35 e 36. Os resultados demonstraram a presença de mucilagens (metabólito primário), esteróides e/ou triterpenos, cumarinas, antraquinonas, flavonoides e taninos (metabólitos secundários) no EHBM. A média da atividade antioxidante relativa do EBHM foi igual a 5,75% e do gel de M. sylvestris 10% foi 26,30%, em relação a 100% do total de atividade antioxidante atribuída ao ácido ascórbico. Não foi observada a formação de halo de inibição tanto para o método da difusão em disco quanto para a técnica da perfuração no ágar na avaliação da atividade antimicrobiana in vitro. No estudo clínico não se observou diferenças estatísticas entre os diferentes tratamentos para os parâmetros clínicos avaliados e a quantidade de fluido gengival. O metabólito primário e os metabólitos secundários presentes na planta sugerem o seu efeito anti-inflamatório. O EHBM apresentou ausência de atividade antimicrobiana e uma reduzida atividade antioxidante.
