Precarização do trabalho no campo obstétrico: percepções de Enfermeiras

Resumo: Many obstetric nurses have been developing practices based on relational technologies in the care of pregnancy, childbirth and after childbirth guided by the humanization movements and neoliberal policies in the 1990s. With the launch of Cegonha Carioca (2011) in the municipal health of Rio de Janeiro, jobs for nurses, inserted in the perspective of opposing the medicalized model of care were increased. At the same time, there are transformations in the management of human resources and care, probably legatoires of the productive restructuring that predicts an increase in productivity and flexibilization of labor relations. This study aimed to: describe the objective conditions of obstetric nurses s work in the hospital field; To analyze the effects of nurses 'working conditions on the obstetric field and to discuss the obstetric nurses' perception of their labor relations. It is an exploratory, descriptive and qualitative research, with 15 obstetric nurses, graduated since 2011, working at the hospital area, at least 6 months ago. The data were collected through a semi-structured interview and treated in two ways: for the first objective, the objective conditions of work were defined based on the thematic reference that originated three categories of the first analytical section. For other objectives, the steps of the method proposed by Minayo (2014) were followed by: transcription of interviews, coding and grouping of statements and creation of four analytical categories. The theoretical framework of Pierre Bourdieu anchored the analysis. The results showed that most of nurses have two employment contracts for the Consolidation of Labor Laws in social organizations. Performing at the same activity, hired nurses have higher salaries than those of the civil servants and smaller ones than those of the contracted obstetrical doctors and the daily work is intensified by the accumulation of functions. Participants perceive instability in the contract bond, leading to insecurity and fear of losing their jobs. The productivist logic pressures by the increase of the number of deliveries and using the "appropriate" instruments to humanize the assistance. The approach to leadership is perceived as a strategy to remain in employment, at the same time as it becomes a meaning of increasing productivity through the reduction of conflicts. Finally, one perceives a hierarchy in the obstetric field that assigns nurses, the less prestigious tasks and reproduces the masculine domination. It is concluded that there is a possible precariousness in labor relationship that is keeping with new ways of production that can impact on the demedicalizer project of the obstetric field and causes withdrawal of the autonomy of women users.

Com o lançamento da Rede Cegonha (2011), na rede municipal de saúde do Rio de Janeiro, aumentaram os postos de trabalho para enfermeiras, inseridas na perspectiva de contrapor o modelo medicalizado de assistência. Simultaneamente, observam-se transformações na gestão de recursos humanos e de cuidados, provavelmente legatárias da reestruturação produtiva que prevê aumento na produtividade e flexibilização dos vínculos trabalhistas. Assim, esse estudo objetivou: descrever as condições objetivas do trabalho das enfermeiras obstétricas no campo hospitalar; analisar os efeitos das condições de trabalho das enfermeiras para o campo obstétrico e discutir a percepção das enfermeiras obstétricas sobre suas relações de trabalho. Trata-se de pesquisa exploratória, descritiva e qualitativa, sendo participantes 15 enfermeiras obstétricas, formadas a partir de 2011, atuantes na área hospitalar, no mínimo, há 6 meses. Os dados foram coletados por meio de entrevista semiestruturada e tratados de duas formas: para o primeiro objetivo, definiram-se as condições objetivas de trabalho a partir do referencial temático que originaram as três categorias da primeira seção analítica. Para os outros objetivos foram seguidos os passos do método proposto por Minayo (2014): transcrição das entrevistas, codificação e agrupamento das falas e criação de quatro categorias analíticas. O arcabouço teórico de Pierre Bourdieu ancorou a análise. Os resultados apontaram que a maioria das enfermeiras pesquisadas possuem dois contratos empregatícios pela Consolidação das Leis Trabalhistas (CLT) em organizações sociais terceirizadas. Na realização da mesma atividade, as enfermeiras contratadas têm salários maiores que os das estatutárias e menores que os dos médicos obstetras contratados e a jornada de trabalho é intensificada pelo acúmulo de funções. As participantes percebem uma instabilidade no vínculo de contrato, levando-as à insegurança e ao medo de perder o emprego. A lógica produtivista pressiona para o aumento da quantidade de partos e o uso dos instrumentos apropriados para humanizar a assistência. A aproximação com a chefia é percebida como uma estratégia para se manter no emprego, ao mesmo tempo em que se torna um meio de incremento da produtividade pela redução de conflitos. Por fim, percebe-se uma hierarquia no campo obstétrico que atribui às enfermeiras as tarefas menos prestigiosas e reproduz a dominação masculina.
