This research aims to address the practice of therapeutic accompaniment (TA) within a psychiatric hospitalization service. It is intent on affirming a clinic in this practice. When it comes to the psychoanalytic work at an infirmary of crisis filled with psychotic patients at a critical condition, significant differences concerning TA intervention must be taken into consideration. TA practice, found within the ward, comes across various barriers: the hospital institutional organization, the routine of the infirmary and TA practice regulated by schedules of daily activities; the primacy of psychiatric care; the dichotomy of good and evil that often appears in the team s behavior and discourse. Such an institutional operation marked by the hierarchy of power and knowledge, filled with defined routinely rules, tends to annihilate the differences of the subject, therefore making their singularities invisible. The therapeutic accompaniment (ta), affected by psychoanalysis, tries to bear loopholes within this institutional setting, within the psychiatric knowledge, within ideals and values of good and evil, of right and wrong and within invisible rules of morality that thrive in the mental hospital. Ta attempts to bear these loopholes, these discontinuities while giving psychotic patients some possible holding, without bearing in mind a truth about them or about what makes them suffer. Psychoanalytic listening proves paramount to the clinical work performed by the at , in his pursuit to make deviations in the institutional functioning and in the various tasks expected to be accomplished. The historical process involving treating madness is delved into, ranging from psychiatric reform to the emergence of the AT. S.Freud's and J. Lacan's theoretical formulations upon psychosis , as well as the concept of the subject, have been used as reference in order to investigate TA practice and its clinical function while accompanying the patients and their details. Transference in psychosis was discussed through the analysis of a clinical case study, emphasizing the role of the secretary of the alienated subject
