Resumo: Human infections caused by Neisseria meningitidis remains a serious public health problem. A vaccine against N. meningitidis serogroup C (MenC) has been shown to be safe and immunogenic in many high-risk populations with varying results depending on the degree of immunosuppression. However, there are few data on the use of this vaccine in children and adolescents infected with HIV. Therefore, the objective of this study was to evaluate the antibody response of patients infected with HIV after immunization against MenC. A prospective cohort study of 154 children and adolescents 2-18 years of age vertically infected by HIV was performed. All subjects received two doses of conjugated vaccine intramuscularly. Blood samples were collected prior to immunization (V1) and 1-2 months (V2), 1 year after immunization (V3) and 1-2 after the booster dose (V4). The bactericidal assay and ELISA was used to evaluate the bactericidal antibody response and IgG response to C polysaccharide, respectively. Two doses of vaccine were required seroconversion of most (70%) of the subjects. There was a marked decrease of bactericidal antibodies and specific IgG one year after the first dose of vaccine. We observed a positive and significant correlations between the results obtained in the bactericidal assay and ELISA after 2 doses of vaccine. The same was seen when we analyze the correlation between the frequency of seroconversion assessed by two tests. We observed a significant correlation between IgG anti-C polysaccharide and SBA titers after the first dose of the vaccine only fort s N79/96, suggesting the influence of the ortho-acetyl radical (OAc), since the C polysaccharide of N79/96 strain is OAc+ while N753/00 is OAc-. This study demonstrated that one dose of vaccine induced immune memory in patients HIV+, which was activated by the second dose of vaccine, which was essential for the seroconversion of most of the patients. It remains important to investigate the best period for the second dose, so that we could have an even greater number of responders in this group of patients.

A vacina conjugada contra N. meningitidis sorogrupo C (MenC) demonstrou ser segura e imunogênica em muitas populações de alto risco, com resultados variáveis de acordo com o grau de imunossupressão. Existem, entretanto, poucos dados sobre o uso dessa vacina em crianças e adolescentes infectados pelo HIV. Portanto, o objetivo deste trabalho foi avaliar a resposta de anticorpos de pacientes infectados pelo HIV após imunização contra o MenC. Foi realizado um estudo de coorte prospectivo com 154 crianças e adolescentes de 2 a 18 anos de idade infectados verticalmente pelo HIV. Todos os indivíduos receberam duas doses da vacina conjugada (Novartis), via intramuscular. As amostras de sangue foram coletadas antes da imunização (V1) e 1-2 meses após (V2), 1 ano após a imunização (V3) e 1-2 após a dose reforço (V4). Utilizamos o ensaio bactericida para avaliação da resposta dos anticorpos bactericidas e o ELISA para avaliar a resposta dos anticorpos IgG anti-polissacarídeo C. Foram necessárias duas doses da vacina conjugada para soroconversão da maioria (70%) dos indivíduos. Houve redução acentuada de anticorpos bactericidas e de IgG específica um ano após a primeira dose da vacina. Registrou-se correlações positivas e significativas entre a resposta de anticorpos bactericidas para as 2 cepas de MenC e níveis de IgG anti-PS-C após 2 doses da vacina. O mesmo foi visto quando analisamos a correlação entre a frequência de soroconversão avaliada pelos dois testes. Observou-se ainda correlação significativa entre IgG anti-polissacarídeo C e títulos de SBA após a primeira dose da vacina apenas para a cepa N79/96, sugerindo a influência do radical orto-acetil (OAc), visto que o polissacarídeo da cepa N79/96 é OAc+ enquanto que a cepa N753/00 é OAc-. Este trabalho demonstrou que uma dose da vacina induziu memória imunológica nos pacientes HIV+, a qual foi ativada pela segunda dose. Esta foi essencial para a soroconversão da maioria dos pacientes.
