Introduction: Low stocks of Human Milk Banks (HMB) observed in some units show the need to analyze human milk donation process. Objectives: to estimate the reliability and the internal consistency of structured questionnaire on human milk donation process. Methods: quantitative study of test-retest reliability and internal consistency of a quantitative questionnaire on donation of human milk with a population of postpartum women with > 18 years of a public maternity hospital in the city of Rio de Janeiro/ RJ. The questionnaire, based on previous qualitative study, investigates the decision-making process of the donation of human milk, divided into five distinct dimensions: learning, contemplation, journey, donation and awareness about the donation. The test was performed in the maternity ward and retesting by telephone after 15 calendar days of the first interview. The agreement was estimated by kappa statistics (dichotomous variables) and weighted kappa (ordinal variables). For internal consistency was used the α-Cronbach coefficient. Results: Were interviewed 79 mothers in the test stage and 37 in the retest (loss = 53,2%). Most of the interviewees were young (20-29 years) with complete elementary school and declared brown or black. In addition, 51,9% reported having visited a HMB and 20,4% of women with more than one child said they had donated their milk. Compliance measures were considered almost perfect for 5,7% of the variables, substantial for 7,6% and moderate for 17,0%. The α-Cronbach questionnaire was 0,79 (high consistency), with the dimensions learning , donation and perception showed better consistency (α= 0,70, 0,90 and 0,78 respectively) and the dimensions contemplation and journey had low consistency (α= -0,05 and 0,39 respectively). Conclusion: Psychometric properties of questionnaire showed good internal consistency and reasonable reproducibility (reliability) because some answers may change over time (mothers can receive new information after hospital discharge). It is recommended the application of this questionnaire in maternity hospitals to evaluate systematically the donation process for HMB
