Resumo: | Acute C hepatitis becomes chronic in most cases. Part of these patients could progress to cirrhosis and develop complications such as: hypertensio, hepatic failure and hepatocellular carcinoma. HIV infection interferes and changes C hepatitis natural history. It is associated to higher levels of viremia and acceleration of the hepatic disease. The main goal of the treatment is to eraticate the virus, avoid the progression to cirrhosis and its complications. This study assess the treatment affectiveness in HIV/HCV co-infected patients using interferon + ribavirin and perginterferon + ribavirin protocols recommended by the Health Minister. The effectiveness ws measured by the HIV/HCV co-infected patients survival, mainly those who have the HCV genotypes 1, 2 e 3 submitted to combination therapy. The Markov model was performed in the review. The thesis present, through his chapters papers about chronic C hepatitis, HCV/HIV co-infectio, co-infected and C hepatitis stages treatment, elements manegement of hepatitis C treatment and the evaluation of the C hepatitis teratment effectiveness. The results shows higer effectiveness using peginterferon. Among genotype 1 patients treated with combined Therapy (peginterferon + ribavirin) 23,2% exibhited response. In genotypes 2 and 3, 49,7% shows response. The treatment with interferon + ribavirin received lower performance, 5,6% genotype 1 patients shows results and 16,2% of genotypes 2 and 3. The re-treatment with peginterferon + ribavirin of the non-responders and relapses presented the following data: 55,8% the response in genotypes 2 and 3 and 13,6% in genotype 1. The survaillance often 10 years of interferon + ribavirin patients was 38,3% for genotype 1 patients and 44,56% genotypes 2 and 3 patients. In perginterferon + ribavirin patients it was 48,58% in genotype 1 and 63,13% in genotypes 2 and 3. The survival rate of retreated patients with peginterferon + ribavirin was 47% genotype 1 patients and 68,17% for genotypes 2 and 3. |

E uma parte desses pacientes pode sofrer progressão até cirrose e apresentar complicações, como hipertensão portal, insuficiência hepática e hepatocarcinoma. A infecção pelo HIV interfere e modifica a história natural da hepatite crônica C e está associada à alta de viremia do HCV e à aceleração da doença hepática. O objetivo principal do tratamento é a erradicação do vírus, para evitar a progressão à cirrose hepática e o desenvolvimento de suas complicações. Este estudo teve como objetivo avaliar a efetividade do tratamento de pacientes co-infectados HCV/HIV com as terapias combinadas interferon + ribavirina e peginterferon + ribavirina recomendadas pelo Ministério da Saúde. A efetividade foi medida pela sobrevivência dos pacientes co-infectados HCV/HIV, particularmente os portadores de HCV genótipo 1 (G1), genótipo 2 (G2) e genótipo 3 (G3) submetidos às terapias combinadas. A análise foi realizada através do modelo de Markov. O trabalho apresenta, em seus capítulos, estudos sobre estágios evolutivos da hepatite C crônica, elementos do gerenciamento do tratamento da hepatite C crônica e avaliação da efetividade das terapias combinadas. Os resultados apontam a maior efetividade dos tratamentos com base no peginterferon. Entre os pacientes de genótipo 1 de HCV tratados com terapia combinada de peginterferon + ribavirina, 23,2% foram responsivos ao tratamento e entre os portadores do genótipo 2, forem responsivos 49,7%. Enquanto o tratamento com interferon + ribavirina obteve resultados mais discretos, 5.6% dos pacientes com genótipo 1 e 16,2% dos pacientes com genótipos 2 e 3 foram responsivos. O retratamento com peginterferon + ribavirina dos pacientes não responsivos e reincididos à terapia com interferon + ribavirina apresentou os seguintes resultados: 55,8% de responsivos entre os portadores de genótipos 2 e 3 e de 13,6% nos portadores de genótipos 1 de HCV. Com relação à sobrevida, após dez anos a taxa de sobrevida dos pacientes tratados com interferon + ribavirina foi de 38,3% para os pacientes com genótipo 1 e 44,56% naqueles portadores de HCV de genótipos 2 e 3; e nos pacientes submetidos ao tratamento com peginterferon + ribavirina foi de 48,58% nos de genótipo 1 de HCV, 63,13% nos pacientes de genótipo 2 e 3 de HCV.
