Morbi-mortalidade nos municípios brasileiros: associação com indicadores sócio-econômicos e de assistência à saúde. Um estudo multinível

Resumo: | Socioeconomic inequity in Brazil has reflections on health sector. The sector's services and complexity distribution is analogous to the general social goods, flowing the country economic development vectos. The size of social disparities observedf makes possible the hypothesis statement of association among socioeconomic and health care indicators and community health outcomes. The analysis of this process requires observation of basic principles of Unique Health System related to territory, as municipality, hierarchy and region-centered policies. The objective of this study was to analyse the association among socioeconomic and health care indicators and some morbidity and mortality indicators. An ecological multilevel study was performed, taking as observation units all Brazilian municipalities for the year 2001. Infectious diseases, neoplasm and circulatori disease mortality rates, and infectious bowel diseases and diabetes mellitus hospitalization rates were obtained. The source of data for these indicators was the Mortality Information System (HIS). In order to obtain more precise rates, and verify the real size of diferences within municipalities, the MIS under-registration was corrected and the deaths and hospital admissions with undefined causes were redistributed, using the James-Stein empirical Bayes estimators for event counts in small areas. The MIS under-registration was corrected in 5,85%, more concentrated in Northern and Northeastern Regions, for neonatal and undefined causes of death, and for extremes of age. The undefined causes of death redistribution has altered the proportional mortality for Northern and Northeastern Regions, while the same procedure for HIS caused no significant changes in hospital proportional morbidity. After morbidity and mortality indicators were corrected by this method, Poisson multilevel regression models were specified for three levels: municipality, mesorregion and State. Results showed, in general terms: (a) variance was identified for second and third levels; (b) socioeconomic and health care variables were associated to all outcomes; (c) predominance of associations for the municipality level; (d) general and high complexity health care infrastructure were directly associated to indicators related to late stage of demographic and epidemiologic transition process; (e) suggestion of adjustment in Health Family Program implementation for its target population, with no impact over the respective epidemiologic profile; (f) suggestion of effects relatyed to politics definition at the municipality policy level over the morbidity and mortality. The implications for observation of these phenomena are discussed under the light of foundation remarks of Unique Health System and its policy-definition present needs. |

A distribuição de serviços e da complexidade do setor apresenta é análoga à dos bens sociais em geral, acompanhando o vetor de desenvolvimento econômico do país. Dada a magnitude das disparidades sociais observadas, é possível levantar a hipótese de que os indicadores de assistência à saúde, assim como os sócio-econômicos, apresentam-se associados a desfechos coletivos em saúde. A análise deste fenômeno exige a observação dos princípios norteadores do Sistema Único de Saúde, relacionados ao território, como a municipalização, a hierarquização e a regionalização. O objetivo deste estudo foi o de analisar a associação entre indicadores sócio-econômicos e de assistência à saúde e alguns indicadores de morbi-mortalidade selecionados. Realizou-se um estudo ecológico multinível, tendo como unidade de observação a totalidade dos munic´pios brasileiros em 2001. Foram obtidas taxas de mortalidade por doenças infecciosas e parasitárias, neoplasias e doenças do aparelho circulatório, e as taxas de internação por doenças infecciosas intestinais e diabetes mellitus. A fonte de dados destes indicadores foram o Sistema de Informações sobre Mortalidade (SIM) e o Sistema de Informações Hospitalares do SUS (SIH-SUS). Para permitir o cálculo mais preciso dos indicadores, de forma a verificar a magnitude real das suas diferenças entre os municípios, corrigiu-se o sub-registro do SIM e redistribui-se os óbitos e internações por causas mal definidas de ambos os Sistemas, através da estimativa de eventos para pequenas áreas baseada nos estimadores bayesianos empíricos de James-Stein. Obteve-se uma correção de sub-registro para o SIM de 5,85%, mais concentrada nas Regiôes Norte e Nordeste, em causas perinatais e mal definidas, e nos extremos de idade. A redistribuição das causas mal definidas de óbito alterou a mortalidade proporcional por causa nas Regiões Norte e Nordeste, enquanto que o mesmo procedimento para o SIH-SUS não promoveu mudanças significativas na morbidade hospitalar proporcional. Após a obtenção dos indicadores de morbi-mortalidade corrigidos por este método, foram especificados, para cada indicador, um modelo de regressão de Poisson em três níveis: município, mesorregião e Unidade das Federação. Os resultados identificaram, em termos gerais: (a) existência de variância para o segundo e terceiro níveis; (b) presença de variáveis sócio-econômicas e assistenciais associadas a todos os desfechos; (c) predominância de associações relacionadas ao nível municipal; (d) a infra-estrutura geral e de serviços de saúde de maior complexidade associadas diretamente aos indicadores relacionados ao estágio mais avançado do processo de transição demográfico-epidemiológica; (e) indícios de adequação da implementação do Programa de Saúde da Família em função de sua população alvo, sem identificação de impacto sobre o perfil epidemiológico correspondente; (f) sugestão de efeitos relacionados à definição política no nível de gestão municipal sobre a morbi-mortalidade.
