This paper aims to analyze the 30 years of the Unified Health System (SUS) and the impacts of World Bank guidelines on Brazilian health policy and in this light to think about how copayment, one of these guidelines, is structured within the State. In this sense, the role of the World Bank in the orientation of health policies in dependent countries is highlighted, generating in Brazil contradictions between the advances of Sanitary Reform in the legal field and the setbacks of an agenda of regressive reforms from the point of view of the working class. The work was structured in three chapters. Initially, a study is carried out on the so-called “cost recovery policies” and copayment in the different historical situations, revealing a duality between the constitutional conquests and the counter-reform agenda, meeting the recommendations of the international financial organizations (IMF and BM). Subsequently, health policy in the 2000s is analyzed, outlining the co-payment during the PT governments until the 2016 institutional coup. Finally, a reflection is made about the Accessible Health Plan proposal, one of the greatest expressions of co-payment in Brazil, proposed by the Temer government, as well as the fight waged by the National Front Against the Privatization of Health in defense of the public SUS, of quality and 100% State. In conclusion, a reflection is made about the current crisis that we are experiencing due to the COVID-19 pandemic, revealing the importance of the State's role in overcoming the capital crisis.
