Obesidade sarcopênica e impacto na capacidade funcional de idosos: estudo FIBRA-RJ

Confluence among obesity and sarcopenic is becoming major public health with consequences in the functionallity of elderly, besides the obesity problem. The first article aimed to evalue the association between obesity sarcopenic and functional disability after four yars of follow-up in elderly. And the second study aimed to assess the prevalence of sarcopenic obesity in elderly, and investigate its association with functionality, lifestyle, biochemical markers and physical morbidity. For the first article we analyzed longitudinal data from 336 elderly health insurance clients and residents in the North of Rio de Janeiro area obtained through questionaires by researchers at the Research Network Fragility in Elderly Brazilians, Rio de Janeiro section (FIBRA-RJ). The collection of the first phase occurred in 2009 and the follow-up in 2013. For the second article we analyzed cross-sectional data from the FIBRA-RJ phase III collected between 2012 and 2013. The results of the first article showed an incidence of functional disability after 4 years of follow-up of 15,2%, higher in women (18,2%; p=0,02), with 4 more comorbidities (22,2%; P=0,04), diabetes mellitus (23,0%; p=0,03) and sarcopenic obesity (32,6%; p=0,001). In multivariate analysis, after adjusting for variables, the relative risk (RR) of disability associated with sarcopenic obesity was 2,8 (95% CI 1,3-5,9). The results of the second article showned that the prevalence of obesity sarcopenic was 29,3% and the variables that showed a statistically significant association with obesity sarcopenic after adjustment for covariates were reduced walking speed (OR=2,43 95% CI 1,09-5,41), self-reported medical diagnosis arthrosis/ arthritis (OR=2,43 95% CI 1,05-5,63), and having higher blood glucose levels (OR=3,14 95% CI 1,17-7,32). As for th first article, we concluded that the incidence of disability in sarcopenic obese elderly is high, with a relative risk about three times higher than among those without condition, regardless of age, sex, diabetes mellitus and number of comorbidity. This finding is important because sarcopenic obesity is a modifiable risk factor of being able to design interventions for the treatment and prevention of this geriatric syndrome. The second papel found high prevalence of obesity sarcopenic is worrying and points to the need to implement public policies to prevent this phenotype of body composition. The associations can assist the development of effective intervention strategies of modifiable factors.
