Avaliação do padrão de sono, resiliência e qualidade de vida de crianças e adolescentes com condições crônicas hospitalizadas

Resumo: Introduction: Children and adolescents with chronic conditions experience significant changes in their daily lives and that of their families, with numerous readaptations and coping strategies, in addition to frequently presenting multiple symptoms, which include sleep disturbances, fatigue, pain, nausea, among others, and compromise their quality of life. Objective: To explore sleep, resilience and health-related quality of life in children and adolescents with chronic conditions during hospitalization. Method: Quantitative, described and cross-sectional study. Participants were children and adolescents with chronic conditions. The instrument used to assess the sleep pattern was the wrist ActiGraph; to assess resilience, the Resilience Scale for Children and Adolescents by Sandra Prince-Embury was used, and to assess quality of life, the Pediatric Quality of Life Inventory; both have valid and reliable Brazilian versions. For data analysis, specific software for data analysis of ActiGraph, descriptive statistics and correlation test were used. Results: 40 children and adolescents between 9 and 18 years old, with chronic conditions, from two reference hospitals in Rio de Janeiro participated. The results showed compromised sleep, with a median sleep duration of 3.7 hours (IIQ = 109.35), median time awake after sleep onset of 39.62 minutes (IIQ = 31.65), percentage of 70.16% sleep (IIQ = 16.35) and sleep efficiency 89.43% (IIQ = 7.90). The scores for total health-related quality of life (Md = 60.86; IIQ = 22.83) and its physical dimensions (Md = 64.06; IIQ = 38.28), emotional (Md = 50.00; IIQ = 25.00) and school (Md = 50.00; IIQ = 31.25) were low. However, resilience levels were classified between medium and high. Correlations were found between health-related quality of life and resilience: the greater the social dimension of health-related quality of life, the greater the resilience control subscale. There was also a correlation between resilience and sleep, showing that the higher the emotional reactivity score, the longer the participants remained awake after falling asleep and the lower their sleep efficiency. In addition, health-related quality of life was influenced by time since diagnosis; resilience, due to age, skin color, diagnosis and other chronic conditions; and sleep, by diagnosis and treatment. Conclusion: Children and adolescents hospitalized with chronic conditions experience significant sleep disorders and have a low quality of life, but have satisfactory levels of resilience. Furthermore, the better the quality of life social functioning, the better the resilience control, and the better emotional reactivity, the participants woke up less after sleep onset and had better sleep efficiency. New studies are needed, especially with larger populations, to develop strategies for managing sleep disorders and favoring quality of life and resilience.

Objetivo: Explorar o sono, a resiliência e a qualidade de vida relacionada à saúde de crianças e adolescentes com condições crônicas durante a hospitalização. Método: Estudo quantitativo, do tipo descrito e transversal. Os participantes foram crianças e adolescentes com condições crônicas. O instrumento utilizado para avaliar o padrão de sono foi o ActiGraph de pulso; para avaliar a resiliência, utilizou-se a Escala de Resiliência para Crianças e Adolescentes de Sandra Prince-Embury, e, para avaliar qualidade de vida, o Inventário Pediátrico de Qualidade de Vida; ambos possuem versões brasileiras válidas e confiáveis. Para análise dos dados, utilizaram-se softwares específicos para análise de dados do ActiGraph, estatístia descritiva e teste de correlação. Resultados: Participaram 40 crianças e adolescentes entre 9 e 18 anos, com condições crônicas, de dois hopitais de referência do Rio de Janeiro. Os resultados mostraram sono comprometido, com mediana de duração de sono de 3,7 horas (IIQ = 109,35), mediana de tempo acordado após início do sono de 39,62 minutos (IIQ = 31,65), percentural de sono de 70,16% (IIQ = 16,35) e eficiência do sono de 89,43% (IIQ = 7,90). Os escores para qualidade de vida relacionada à saude total (Md = 60,86; IIQ = 22,83) e suas dimensões física (Md = 64,06; IIQ = 38,28), emocional (Md = 50,00; IIQ = 25,00) e escolar (Md = 50,00; IIQ = 31,25) foram baixos. No entanto, os níveis de resiliência foram classificados entre médio e alto. Foram encontradas correlações entre qualidade de vida relacionada à saúde e resiliência: quanto maior a dimensão social da qualidade de vida relacionada à saúde, maior a subescala de controle da resiliência. Também houve correlação entre resiliência e sono, mostrando que quanto maior o escore da reatividade emocional, mais tempo os participantes permaneciam acordados após início do sono e menor a eficiência do sono. Além disso, a qualidade de vida relacionada à saúde foi influenciada pelo tempo de diagnóstico; a resiliência, pela idade, pela cor de pele, pelo diagnóstico e por outras condições crônicas; e o sono, pelo diagnóstico e pelo tratamento. Conclusão: Crianças e adolescentes hospitalizados com condições crônicas vivenciam importantes distúrbios do sono e possuem baixa qualidade de vida, mas apresentam níveis satisfatórios de resiliência. Além disso, quanto melhor o funcionamento social da qualidade de vida, melhor o controle da resiliência, e quanto melhor reatividade emocional, os participantes acordaram menos após o início do sono e tiveram melhor eficiência do sono.
