Significando a transicionalidade da sexualidade do cônjuge-cuidador da pessoa idosa em processo demencial para um cuidado terapêutico de enfermagem

Sexuality includes many dimensions over subjects´ lives. In a lifetime it has been revised, as biopsychosocial changes take place and, when marital long-term relationships are taken into consideration it is important to consider that these couples have experienced changes in their marital and family relationships. For the elderly the prevalence of chronic diseases is higher, highlighting the risk for disability and dependency, among them dementia. Spouses-caregivers experiencing sexuality transitions can give a new meaning to life, with the support of the therapeutic nursing care. This study aimed to understanding the meanings of sexuality established by the spouses-caregivers of the elderly in dementia process in order to prepare an interpretive model of nursing care under the therapeutic perspective of the Theory of Transitions. It has a qualitative approach, of exploratory and descriptive nature, under the methodological reference of the Grounded Theory. The study was carried out with 25 subjects who were spouses-caregivers, children and health professionals, most of them, nurses, split into four sample groups. The setting of the investigation was the Care Center for the Elderly, Rio de Janeiro, RJ, Brazil. Data collection occurred from April 2014 to April 2015. The collection technique was the intensive interview and the analysis performed upon initial, selective and focused coding. The research project was approved by the Ethics Committee of UERJ (nº 631538). Interview data showed that the construction of marriage over people´s lives and the previous history of sexuality perceived as well as experienced can influence the way to identify the changes caused by dementia process and how to adapt to the consequences for personal and marital sexuality. It was evidenced spouses-caregivers´ new meaning to life concerning the elderly under dementia process by means of transitionality of couple´s sexuality, which leads to therapeutic nursing care, supported by seven categories and sixteen subcategories, as they, altogether, point to such a new meaning straightly linked to the marital history, family engagement as a support, besides specialized services and the will to play new dimensions for the sexual life from the part of the subject himself and the beloved one. The results scored that care comes as another assignment, performing health and other dimensions changes. From the professionals, there is still lack of knowledge and a very fair approach concerning sexuality among elderly couples, greatly in the context of dementia beliefs and the social imaginary interfere not only in the development of sexuality possible in the context of current life, but also in the care relationship and define what their social role is; sons and daughters describe family functioning in the pursuit of adaptation to the new life context of the parents. The reframing exists, from coping strategies and the implementation of sexuality for the care to demented spouse. Thus, the thesis is sustained: Understanding the spouse-caregivers sexuality transitions experience regarding the elderly under dementia process allows the construction of an interpretive model which highlights a particular therapeutic nursing care for that moment in their lives.
