Resumo: | Sickle cell anemia (SCA) is a hemoglobinopathy characterized by inflammation, vaso-occlusion phenomena and chronic hemolysis. Hepcidin is the hormone mainly responsible for iron homeostasis and intestinal absorption. Recents studies have observed that iron load influences in serum hepcidin concentrations. To establish a safe nutritional approach, the estimation of the iron absorption capacity needs to be better understood, especially for children, whose have a high rate of growth and development. Evaluate the iron absorption capacity in children with sickle cell anemia. Experimental study, carried out with 14 children (Median = 8.0 years; Interquertile range: 7 - 9 years). Three groups participated in the study: the control group, comprised of individuals without SCA (n=2); those with the disease but without iron overload (n=8); and those with the disease and iron overload (n=4). The evaluation of the iron absorption capacity was performed by marking a standardized food (100 mL of orange juice) with the stable isotope 57Fe (5 mg). Blood samples were colected at baseline and 14 days after ingestion of the isotope and the isotopic relationship between 56Fe/57Fe was provided by inductively coupled plasma mass spectrometry (ICP-MS). In the group of children with SCA without iron overload, a moderate positive correlation was found between ferritin and hepcidin (r = 0.762; p <0.05) and a strong negative correlation between ferritin and 57Fe absorption percentage (r = - 0.962; p <0.05). In the same group, the HbS percentual was strongly correlated with the percentage of iron absorption (r = 0.976; p <0.05). The percentage of 57Fe absorption did not correlate with hepcidin and liver iron concentration in any of the groups. The 57Fe absorption percentage was statistically different among the three groups studied (3.9%, children with SCA and iron overload; 18.2%, children with SCA and without iron overload; and 12.67%, children without the disease). The percentage of intestinal iron absorption found in children with SCA and iron overload suggests that the recommendations for restriction of iron source foods, to which these individuals are frequently submitted, may not be necessary. |

Fatores intrínsecos à doença podem influenciar de forma positiva ou negativa na síntese da hepcidina, hormônio responsável pela homeostase de ferro. A eritropoiese intensa e a anemia hemolítica podem ocasionar a redução deste hormônio, acarretando aumento da absorção intestinal deste micronutriente. No entanto, a sobrecarga de ferro transfusional observada em alguns pacientes, assim como o quadro inflamatório, poderiam levar ao aumento da produção de hepcidina e, consequentemente, reduzir a absorção intestinal de ferro. Estudos clínicos realizados em adultos, observaram que as concentrações séricas deste hormônio diferiram em função da carga de ferro, tendo os participantes com sobrecarga deste mineral e AF apresentado maiores concentrações de hepcidina se comparados aos participantes com AF sem sobrecarga e aos controles sem a doença. Estes dados sugerem que os indivíduos expostos à carga excessiva de ferro não precisariam reduzir a ingestão de alimentos ricos neste micronutriente, orientação comumente prescrita por profissionais de saúde. Contudo, estes resultados ainda não são suficientes para estabelecer uma abordagem nutricional segura para estes indivíduos, visto que a estimativa da capacidade de absorção de ferro precisa ser melhor compreendida. Estas informações são importantes especialmente para as crianças, porque a redução do consumo de determinadas fontes alimentares como carnes e feijões, pode levar à deficiência de micronutrientes fundamentais para o seu crescimento e desenvolvimento. Assim, o objetivo deste estudo é avaliar a capacidade de absorção de ferro em crianças com anemia falciforme. Este é um estudo experimental, realizado com 14 crianças (idade mediana: 8,0 anos; intervalo interquartil: 7 – 9 anos) divididas nos seguintes grupos e pareadas por sexo, cor/raça e idade: controle, composto por indivíduos sem a doença (n=2); AF sem sobrecarga de ferro (n=8); e AF com sobrecarga de ferro (n=4). A avaliação da capacidade de absorção de ferro foi realizada por meio da marcação de um alimento padronizado (100 mL de suco de laranja) com o isótopo estável 57Fe (5 mg). Amostras de sangue foram obtidas no tempo basal e 14 dias após a ingestão do isótopo, sendo a relação isotópica entre 56Fe/57Fe determinada por espectrometria de massa com plasma indutivamente acoplado (ICP-MS). O percentual de absorção de 57Fe foi estatisticamente diferente entre os três grupos estudados (3,9%, crianças com AF e sobrecarga de ferro; 18,2%, crianças com AF e sem sobrecarga de ferro; e 12,7%, crianças sem a respectiva doença). No grupo de crianças com AF sem sobrecarga de ferro foi encontrada correlação positiva de grau moderado entre ferritina e hepcidina (r = 0,762; p < 0,05) e correlação negativa de grau forte entre ferritina e percentual de absorção de 57Fe (r = -0,962; p < 0,05). No mesmo grupo, o percentual de HbS foi fortemente correlacionado com o percentual de absorção de ferro (r = 0,976; p < 0,05). O percentual de absorção de 57Fe não se correlacionou com a hepcidina e com a concentração hepática de ferro em nenhum dos grupos. Possivelmente, devido ao pequeno número amostral do grupo com sobrecarga, não foi encontrada correlação entre absorção de ferro e ferritina.
