This is a study about the changes in delivery care at the Municipal Women's Institute Fernando Magalhães (IMMFM) during the implementation period of high-risk assistance. The objective of this study is to analyze the relationship of forces bet ween doctors and nurses during the insertion process and to discuss the strategies of obstetric nurses' struggle to occupy the hospital delivery room. The initial mark of 1988 is justified because it is the year of implementation of the obstetric nurse's intervention project in the delivery of IMMFM, of the Municipal Health Department. The final milestone of 1992 was selected as the year in which the Nursing Service of this Institution was occupied by an obstetric nurse from this same project. This is a qualitative study of a social-historical nature. Phonal and written documents were used as primary sources. The analysis of the primary sources followed the Bardin content analysis method. This study was based on the concepts of field, capital, symbolic power and habitus of Pierre Bourdieu. The results showed the strength games present in the field and in the strategies of obstetric l nurses' struggle to occupy the hospital obstetric field. It was concluded that the insertion and occupation of birth attendance espaces by obstetric nurses meth institutional interests of the medical group in an attempt to reduce the rates of perinatal asphyxia during the process of implementing high-risk assistance and also served the interests of the group of nurses who fought to act in the direct assistance to the hospital birth. In the historical context, the occupation of labor delivery spaces by a non-medical agent interrupted medical hegemony in child birth care and led to a transformation in the Field of hospital parturition in the institution, which later resulted in the implementation of the humanized model of assistance to hospital delivery.
