A representação social do cuidado ao paciente soropositivo ao HIV/Aids entre profissionais de saúde

Resumo: This research aims at identifying structural representation among health professionals of health care provided to HIV/AIDS-infected patients; describing the representation of health care on the basis of structure and process approaches; analyzing health care on the basis of the representations depicted by the health team; and discussing changes in knowledge and practice derived from those representations. Qualitative, descriptive, and exploratory research conducted within the Social Representations theoretical frameworks. Subjects of research were comprised of health professionals working in state-run health institutions in Rio de Janeiro, RJ, Brazil. One hundred forty-eight (148) professionals of twenty (20) health institutions participated in the first stage of the research, and twenty-three (23) professionals of two (2) health institutions participated in the second one. Data collection was conducted on the basis of characterization questionnaires, free evocation, and semi-structured interviews. Data analysis used the four-folded frame run on the EVOC 2003 software; similitude and lexical analyses were accomplished on the basis of the Alceste 4.10 software. As for the structure of the representation, its Central Nucleus was integrated by love, welcome, information, respect, adhesion-treatment, and non-discrimination, all of which signaling with affective, pragmatical, and functional aspects of that dimension of care. Within the contrasting zone, protection-professional stands out. Similitude analysis identified the central stand of the term love. It organizes representation and is strongly related to the term solidarity. The Alceste-run analysis defined six classes, as follows: the constitution of the health team and the care process: definitions, practices, and difficulties; protective practices adopted by health professionals in face of HIV; professional attitudes; evaluations and judgment; social memories about AIDS: origin, circulation, information, and images; and, finally, the AIDS epidemic today. Conclusions show that social representation of care among health professionals is structured and organized on the basis of an interaction sustained by an archeology of the so-called noble feelings, as well as abnegation and surpassing actions. Love stands out at its core, as it characterizes actions and conditions praxis of care. The relevance of that loving care signals to the humanizing of care actions, as they can both meet the demands of individuals during manifestation of the disease, and respect their uniqueness as human beings, facilitating their condition and their search for health improvement.

Pesquisa qualitativa, descritiva e exploratória, orientada pela Teoria das Representações Sociais. Os sujeitos do estudo foram profissionais de saúde atuantes em instituições públicas no Rio de Janeiro, sendo na primeira etapa, 148 profissionais de 20 instituições e, na segunda, 23 de duas instituições. A coleta de dados deu-se através de questionários de caracterização, evocações livres e entrevista semiestruturada. A análise dos dados utilizou o quadro de quatro casas a partir do software EVOC 2003, a análise de similitude e a análise lexical pelo software Alceste 4.10. Com relação à estrutura da Representação, destaca-se que o Núcleo Central foi composto por: amor, acolhimento, informação, respeito, adesão-tratamento e não-discriminação, que indicam uma dimensão relacionada aos aspectos afetivos, práticos e funcionais do cuidado. Na zona de contraste, ressalta-se a presença da proteção-profissional. A partir da análise de similitude foi possível perceber a tendência à centralidade do léxico amor. Ele organiza a representação e possui forte ligação com o termo solidariedade. Na análise realizada pelo Alceste, foram definidas seis classes: A constituição da equipe de saúde e o processo de cuidar: definições, práticas e dificuldades; As práticas de proteção adotadas pelos profissionais frente ao HIV; As formas de enfrentamento diante do diagnóstico positivo ao HIV; Atitudes profissionais: avaliações e julgamentos; As memórias sociais sobre a aids: origem, circulação, informação e imagens e, por fim, A epidemia de aids nos dias atuais. Conclui-se que a representação social do cuidado entre profissionais de saúde tem, em sua estrutura e organização, a interação sustentada por uma arqueologia que se liga aos considerados sentimentos nobres e às ações de abnegação e de superação humanas, tendo como elemento central o amor que caracteriza suas ações e influencia sua práxis.
