Incidências do real em um hospital de cardiologia: apontamentos a partir de uma experiência

This dissertation emphasizes the incidences of the real in a hospital of cardiology. It takes its bases on Sigmund Freud's and of Jacques Lacan's theoretical approaches, including the contributions of others authors in psychoanalysis. It points, first of all, to the three registers indicated by Lacan the symbolic, the imaginary and the real with emphasis on the real, which appears frequently in the hospital context. The real is defined as what escapes from the symbolic, what is non sense and can become traumatic. The concept of trauma is considered as an excess of energy that invades the psychical apparatus, without the subject's preparation to account for it. Anguish, fantasies and humor are studied as possible protections against the real. We verified that the constant presence of anguish and death in the hospital testify that the the real is there and insists to appear the anguish as an affection that appears when the subject meets the real, and death as a figure of the real, the one that points to the impossible. In front of a danger, for example, as the eminence of death, many patients attended by me in the INC (National Institute for Cardiology) are taken by anguish. They are constantly facing events of the order of the horror, the inbearable. The present text discusses also the medical discourse, that is predominant in the hospital, and the analyst's discourse, with a focus on the different manners to deal with the presence of the real in the hospital. All the dissertation along we present fragments of clinical cases in order to associate practice with the theory of Freud and Lacan.
