Violência física entre parceiros íntimos e práticas alimentares de lactentes

Resumo: This thesis involves two related studies. The first study aims to give theoretical support to the main study, a systematic review of the literature on intimate partner violence (IPV) and the feeding practices of children. The second article aims to examine the association of physical intimate partner violence (PIPV) and the minimum complementary feeding (MCF) of breastfed children (BF) and children on artificial feeding (AF). A systematic review of observational studies was conducted in the databases LILACS, MEDLINE, Scopus, Psychoinfo and Science Direct. Inclusion criteria were original observational studies which investigated the forms of emotional IPV, physical and / or sexual and feeding practices of children under two years. The susceptibility to bias of the studies was assessed by individual items from particular form developed. It included 10 articles, eight cross-sectional studies, case-control and cohort. Almost all of the studies investigated breastfeeding. The IPV was related to inadequate feeding practices in infancy in most of the studies found. To reach the second was a cross-sectional study of women and children in the first year of life, who attended four health units in the city of Rio de Janeiro, between June 2005 and July 2008. Information on the PIPV was obtained through the Portuguese version of the CTS-1 instrument (Conflict Tactics Scales-Form R). Complementary food supply was obtained from the instrument with closed questions for the last 24 hours applied between the twelfth month and fifteenth month of life of children. The study results show that the offer of MCF among breastfed children was only 11% and the MCF among children who only receive artificial milk was 41%. The violence of physical, less severe and severe were observed in 26.7%, 25.8% and 11.1% of couples, respectively. The PIPV was significantly associated in the analyzes adjusted to not offer the MCF among children who receive breast milk and to offer MCF among children who receive formula. In homes where couples physically attack there is a greater chance of not offering breast milk, infant formula of offering and make use of bottles compared with homes where violence was not reported. Physical violence between couples is associated with feeding practices of breastfeeding and complementary feeding. A closer look at health professionals to aspects related to these phenomena can assist the family and in the decisions of the victim of violence, how to deal with the conflict situation, their autonomy and their relationship with the family's eating habits

O primeiro trabalho visa dar suporte teórico ao estudo principal, uma revisão sistemática da literatura sobre a violência entre parceiros íntimos (VPI) e as práticas alimentares de crianças. O segundo artigo tem como objetivo examinar a associação da violência física entre parceiros íntimos (VFPI) e a alimentação complementar mínima (ACM) de crianças em aleitamento materno (LM) e de crianças em aleitamento artificial (LA). A revisão sistemática de estudos observacionais foi realizada nas bases de dados LILACS, MEDLINE, SCOPUS, PsicoINFO e Science Direct. Os critérios de inclusão foram estudos originais do tipo observacional os quais investigaram as formas de VPI emocional, física e/ou sexual e as práticas alimentares de crianças menores de dois anos. A suscetibilidade a vieses dos estudos foi avaliada por itens individuais a partir de formulário designadamente desenvolvido. Foram incluídos 10 artigos, sendo oito estudos do tipo transversal, um caso-controle e um de coorte. Quase a totalidade dos estudos investigou o aleitamento materno. A VPI esteve relacionada às práticas alimentares inadequadas na infância na maioria dos estudos encontrados. Para alcance do segundo foi realizado um estudo transversal com mulheres e crianças no primeiro ano de vida, que compareceram a quatro unidades de saúde do município do Rio de Janeiro, entre junho de 2005 e julho de 2008. Informações referentes à VFPI foram obtidas por meio da versão em português do instrumento CTS-1 (Conflict Tatics Scales Form R). A oferta de alimentação complementar foi obtida a partir do instrumento com perguntas fechadas para as últimas 24 horas aplicados entre o décimo segundo mês e o décimo quinto mês de vida das crianças. Os resultados do estudo mostram que a oferta da ACM entre crianças em aleitamento materno foi de apenas 11 % e da ACM entre crianças que só recebem leite artificial foi de 41 %. As violências de forma física, menos grave e grave foram observadas em 26,7 %, 25,8 % e 11,1 % dos casais, respectivamente. A VFPI foi associada significativamente nas análises ajustadas a não ofertar a ACM entre crianças que recebem o leite materno e a ofertar ACM entre crianças que recebem leite artificial.
