O risco biológico no cuidado de enfermagem junto aos clientes submetidos à hemodiálise: estudo sociopoético

Resumo: Viral infections affect a significant portion of individuals in renal replacement therapy (RRT) and pose a significant risk to the health of practitioners when they do not adopt safe practices. The object of study of this dissertation was the Behavior of the nursing professionals against the risk of accident with biological material in the care of clients submitted to hemodialysis. From the guiding question: How are the biological risk and the standard precautionary measures perceived by the nursing professionals who attend clients on hemodialysis? The objectives were formulated: Analyze the imaginative dimension of nursing professionals regarding the risk of biological accident in the care of clients on hemodialysis; understand their perception of risk; describe adherence to standard precautionary equipment; and to discuss self-care behaviors in the light of Nola Pender's Health Promotion Model Theory. This theory was chosen as a theoretical framework, sociopoetical philosophy and theory as a theoretical-methodological framework. After submission and approval by the Research Ethics Committee number CAAE 61649716.6.0000.5282 and Opinion Number: 1,858,402; the research group (GP) was instituted, composed by 10 participants. The data were produced in six sociopoetic workshops, lasting two hours each, in April 2017, in Rio de Janeiro. Through the completion of a professional sociodemographic questionnaire and the use of the artistic techniques Geomantic Experience and Body Dynamics as Minimum Territory. It was evidenced that the GP was composed by 08 women and two men, being 08 nurse technicians and two nurses. 05 reported having already suffered an accident with biological material, all of which were with cutting material; six participants reported that at some point they stopped using PPE; being that the six reported that they had PPE available for use and justified fail to use due to hurry, believing that the absence of gloves would allow a better tact and the devaluation of the risk of contamination. The GP acknowledges the presence of biological risk, the possibility of accident in its daily doing and that the accident may be associated with individual responsibility for factors such as human condition and divine will. The accident is perceived as likely to happen, and is associated with the importance of knowing the factors to which it is associated, the need for adoption and correct use of PPE, keeping up to date and behavior change. The discussion of the behavior of the professionals mediated by the Theory of Health Promotion Model revealed the interfering and motivating factors of the self-care and evidenced the intention of the GP to adopt behaviors and attitudes directed to the promotion of health. It was concluded, therefore, that the nursing team needs to be guided on the importance of preventive actions (self-care), because although some had knowledge about biological risk and self-care this did not always translate into safe behavior. Evidencing the need to conduct courses and training on the topic investigated in this dissertation.

O objeto de estudo desta dissertação foi o Comportamento dos profissionais de enfermagem frente ao risco de acidente com material biológico no cuidado de clientes submetidos à hemodiálise. A partir da questão norteadora - Como o risco biológico e as medidas de precaução padrão são percebidos pelos profissionais de enfermagem que atendem aos clientes em hemodiálise? foram formulados os objetivos: analisar a dimensão imaginativa dos profissionais de enfermagem referente ao risco de acidente biológico no cuidado junto aos clientes em hemodiálise; compreender a percepção destes profissionais frente ao risco; descrever a adesão aos equipamentos de precaução padrão; e discutir os comportamentos de autocuidado à luz da Teoria do Modelo de Promoção da Saúde de Nola Pender. Escolheu-se esta Teoria do Modelo de Promoção da Saúde como marco teórico, e a filosofia e Teoria Sociopoética como marco teórico- metodológico. Após submissão e aprovação pelo Comitê de Ética em Pesquisa número CAAE 61649716.6.0000.5282 e Parecer Consubstanciado número: 1.858.402; foi instituído o grupo-pesquisador (GP), composto por 10 participantes. Os dados foram produzidos em seis oficinas sociopoéticas, com duração de duas horas cada, no mês de abril de 2017, no Rio de Janeiro - Brasil, através do preenchimento de um questionário sociodemográfico e profissional e da utilização das técnicas artísticas Vivência de Lugares Geomíticos e Dinâmica do Corpo como Território Mínimo. Evidenciou-se que o GP foi composto por 08 mulheres e 02 homens, sendo 08 técnicos de enfermagem e dois enfermeiros. 05 participantes referiram ter sofrido acidente com material biológico, todos foram com material perfuro cortante; 06 participantes referiram que deixaram em algum momento de usar EPI; sendo que, os 06 referiram que tinham EPI à disposição para uso e justificaram a não adesão pela pressa, crença que a ausência de luvas possibilitaria um melhor tato e a desvalorização do risco de contaminação. O GP reconhece a presença do risco biológico, a possibilidade de se acidentar e que o acidente pode estar associado além da responsabilidade individual a fatores como a condição humana e a vontade divina. O acidente é percebido como passível de acontecer, e surge associado à importância do conhecimento dos fatores aos quais está associado, a necessidade da adesão e utilização de forma correta dos EPI, de manter-se atualizado e mudança de comportamento. A discussão do comportamento dos profissionais mediada pela Teoria do Modelo de Promoção da Saúde descortinou os fatores interferentes e motivadores do autocuidado e evidenciou a intenção do GP em adotar comportamentos e atitudes direcionados à promoção da saúde. Concluiu-se, então, que a equipe de enfermagem necessita ser orientada sobre a importância das ações preventivas (autocuidado), pois muito embora, alguns possuíssem conhecimento sobre risco biológico e autocuidado este nem sempre se traduzia em comportamento seguro.
