O cuidado de enfermagem ao familiar acompanhante da criança com câncer em cuidados paliativos: um olhar fenomenológico

Resumo: The practice of the nursing team regarding the child with cancer and their family includes care at various levels of health care that include: planning and implementation of adequate interdisciplinary team interventions. These types of care are preventive, curative, and palliative. Curative care consists of the diagnosis, treatment and control of the cancer; with the worsening of the disease comes the palliative care. In this context, it is important to take care of the child, including the family, caring about their reactions and feelings. Thus, the following guiding question was drawn: What are the expectations of the family members accompanying the child with cancer when they seek the nursing team in the context of palliative care? Object of study: the expectation of the family member accompanying the child in palliative care about the care provided to him by the nursing team. Objectives: To understand the meaning of nursing care for the family member accompanying the child with cancer in the context of palliative care; To describe the care provided by the nursing team to the family member accompanying the child with cancer in the context of palliative care; To comprehensively analyze the expectations of the family member accompanying children with cancer when they seek the nursing team in the context of palliative care. Methodology: This is a qualitative study based on Alfred Schütz s sociological phenomenology through the in-order-to motives and because motives from the apprehension of meanings of the family members lived experiences. The study was conducted after approval by the Ethics and Research Committee of the State University of Rio de Janeiro (UERJ) under Opinion No. 2.250.573 and by the Ethics and Research Committee of the National Cancer Institute (INCA) under Opinion No. 2.289.477. Results and discussion: We identified five concrete categories of the lived experiences of the family members accompanying children with cancer in palliative care, described as: a) From the weaknesses in the healthcare scenario to being able to the possibility of feeling cared for when they learn about the palliative care provided to their children; b) To accept the palliative care carried out by the nursing team and the multiprofessional team to facilitate the emotional confrontation in the family member's experience; c) To point out the special care provided by the nursing team in order to obtain tranquility and trust; d) To receive the nursing team s and other professionals care through affection and attention, promoting humanization; e) To seek their faith to experience their child s illness, which in the eyes of the professionals is incurable. Final considerations: This study allowed us to apprehend the family members expectations about the care they receive from the nursing team; these expectations are: multidisciplinary approach with the professionals that assist the child and their family in palliative care, especially the nursing team present in the 24 hours providing this assistance conducting daily technical care. It is necessary to create strategies to include the family member accompanying the child with cancer under palliative care during hospitalization, stimulating their presence and making oneself available so that family members can express their anguish and limitations; therefore, the family member feels cared as care is provided to their children.

Esses cuidados são preventivos, curativos e paliativos. O cuidado curativo consiste no diagnóstico, tratamento e controle do câncer; com o agravamento da doença, chega-se ao cuidado paliativo. Neste contexto, é importante cuidar da criança, incluindo os familiares, preocupando-se com suas reações e sentimentos. Assim sendo, traçou-se como questão norteadora: Quais as expectativas dos familiares acompanhantes de crianças com câncer quando buscam a equipe de enfermagem no contexto dos cuidados paliativos? Objeto de estudo: a expectativa do familiar acompanhante da criança em cuidados paliativos sobre o cuidado prestado a ele pela equipe de enfermagem. Objetivos: Compreender o significado do cuidado de enfermagem para o familiar acompanhante da criança com câncer no contexto dos cuidados paliativos; Descrever o cuidado prestado pela equipe de enfermagem ao familiar acompanhante da criança com câncer no contexto dos cuidados paliativos; Analisar compreensivamente as expectativas dos familiares acompanhantes de crianças com câncer quando buscam a equipe de enfermagem no contexto dos cuidados paliativos. Metodologia: Trata-se de estudo qualitativo pautado na fenomenologia sociológica de Alfred Schütz através dos motivos para e porque da apreensão dos significados do vivido desses familiares. Foi realizado após a aprovação do Comitê de Ética e Pesquisa da Universidade do Estado do Rio de Janeiro (UERJ) sob o Parecer nº 2.250.573 e do Comité de Ética e Pesquisa do Instituto Nacional de Câncer (CEP INCA) sob o Parecer nº 2.289.477. Resultados e discussão: Foram identificadas cinco categorias concretas do vivido dos familiares acompanhantes de crianças com câncer em cuidados paliativos, descritas como: a) Das fragilidades nos cenários de saúde à possibilidade de se sentir cuidado, através do aprendizado sobre os cuidados paliativos prestados às suas crianças; b) Aceitar o cuidado paliativo realizado pela equipe de enfermagem e equipe multiprofissional para facilitar o enfrentamento emocional na vivência dos familiares; c) Destacar os cuidados especiais prestados pela equipe de enfermagem para obter tranquilidade e confiabilidade; d) Receber os cuidados da equipe de enfermagem e de outros profissionais por meio de carinho e atenção promovendo humanização; e) Buscar a fé para vivenciar a doença da minha criança que aos olhos dos profissionais é incurável. Considerações finais: Este estudo possibilitou apreender a expectativa dos familiares acerca do cuidado recebido por eles através da equipe de enfermagem; as expectativas são: a abordagem multidisciplinar com os profissionais que atendem a criança e sua família no contexto dos cuidados paliativos, especialmente a enfermagem, que está presente nas 24 horas prestando esta assistência através dos cuidados técnicos que são realizados diariamente.
