Violências, relações afetivas e drogas: estudo com adolescentes em acolhimento institucional no Rio de Janeiro/RJ

Resumo: | This study aims to analyze the association between alcohol and other drugs use and abuse with manifestations of violence in intimate relationships of sheltered adolescents. And as specific objectives: to characterize the sociodemographic profile of sheltered adolescents who experienced intimate relationships; to estimate the prevalence of use and abuse of alcohol and other drugs by sheltered adolescents who experienced intimate relationships; and to identify the manifestations of violence in the intimate relations of sheltered adolescents. It is a descriptive, exploratory study of the sectional type with a quantitative approach. The research was carried out in all public institutional shelter units for adolescents in the city of Rio de Janeiro (SMAS / RJ). Participants were 72 adolescents of both sexes, between 12 and 18 years old, sheltered between april and june 2017 who had already had an intimate relationship. A multidimensional form previously validated and transculturally adapted instruments (PAJ, ASSIST2.0, CADRI) was applied individually, and the data were analyzed through the descriptive statistics technique firstly through tabulation in the EpiData software and a later export to the STATA Software SE13 for cleaning, processing and analysis. In accordance with Resolution 466/2012 of the National Health Council, the survey was approved by the CEP number 1.779.288. The data were organized into three categories: 1. Characterization of the sociodemographic profile of adolescents in the city of Rio de Janeiro; 2. Interpersonal and contextual relations of sheltered adolescents in the city of Rio de Janeiro; 3. Use of alcohol and illicit drugs and its interface with violence in intimate relationships. The first category shows the reality of the sheltered adolescents, in majority poor, black and drug users, which shows the vulnerability which they are exposed by the distance of family and social ties, stigmas and social exclusion that legitimize structural violence. In the second category, the data show the contexts of the social and family environments in which school dropouts, sexual prejudice and antisocial and risk behaviors which relates to the infraction practices, violence, daily interaction with violence in intimate relationships and drug use associated to the peer pressure and family. The third category shows evidence of alcohol and drug use for the majority of adolescents, concomitant with the process of social learning through family and peer relationships. In addition, there is an intensification of vulnerability to the occurrence of violence in intimate and affective relationships, involving relational, sexual and physical violence in a symmetrical manner and mainly perpetrated by female adolescents. Promoting measures to prevent violence, alcohol and other drugs, and health promotion is important in health care systems, schools and shelters for the development of healthy relationships, also that would be adapted to the characteristics and needs of the sheltered adolescents. |

E como objetivos específicos: caracterizar o perfil sociodemográfico dos adolescentes em acolhimento institucional que vivenciaram relações afetivas intimas; estimar a prevalência do uso e abuso de álcool e outras drogas por adolescentes em acolhimento institucional que vivenciaram relações afetivas intimas; e identificar as manifestações de violências nas relações afetivas intimas de adolescentes em situação de acolhimento. Trata-se de um estudo descritivo, exploratório do tipo seccional com abordagem quantitativa. A pesquisa foi realizada em todas as unidades públicas de acolhimento institucional para adolescentes no município do Rio de Janeiro (SMAS/RJ). Participaram 72 adolescentes acolhidos de ambos os sexos, entre 12 e 18 anos no período de abril a junho de 2017 que já tinham vivenciado relação afetiva íntima. Aplicou-se um formulário multidimensional com instrumentos (PAJ, ASSIST2.0, CADRI) validados e adaptados transculturalmente de maneira individual, e os dados foram analisados através de Estatística descritiva através da tabulação no software EpiData e exportação ao Software STATA SE13 para limpeza, processamento e análise. Em conformidade com a resolução 466/2012 do CNS, a pesquisa foi aprovada pelo CEP nº1.779.288. Os dados foram organizados em três categorias: 1.Caracterização do perfil sociodemográfico de adolescentes em acolhimento no município do Rio de Janeiro; 2. Relações interpessoais e contextuais de adolescentes acolhidos no município do Rio de Janeiro; 3. Uso de álcool e drogas ilícitas e sua interface com a Violência nas relações afetivas intimas. A primeira categoria mostra a realidade dos adolescentes acolhidos, maioria negros, pobres e usuários de drogas, que traz à luz a vulnerabilidade diante do desgastes dos laços familiares e sociais, dos estigmas e exclusão social legitimados na violência estrutural. Na segunda, os dados mostram contextos dos meios social e familiar em que se evidenciam a evasão escolar, discriminação sexual e comportamentos de risco e antissociais relacionados às práticas infracionais, violência, convívio com violência nas relações afetivas e drogadição atrelados ao relacionamento de pares e familiar. A terceira traz o uso de álcool e drogas para a maioria dos adolescentes, concomitante ao processo de aprendizagem social. Há intensificação da ocorrência de violência nas relações afetivas íntimas, envolvendo violência relacional, sexual e física de maneira simétrica e principalmente perpetrada por adolescentes femininas.
