Resumo: Chronic kidney disease (CKD) can lead to muscle mass wasting and sarcopenia. The inflammatory status inherent to the disease, the dialysis, especially the hemodialysis (HD) and the changes in muscle mass related to aging can partially explain the increased protein catabolism present in CKD. Several studies demonstrated the usefulness of computed tomography (CT) images for the assessment of muscle mass as a reference method. However, in the CKD patients, few studies applied CT aiming the assessment of muscle mass. The present study aims to evaluate the muscle mass by reference methods and to describe the prevalence of sarcopenia in two cohorts of patients with CKD, one comprised by patients on CKD stages 3 to 5 (non-dialysis dependent) and another comprised by elderly patients on HD. Considering CT as a reference method, the aim of the first paper was to evaluate the muscle mass in patients with CKD stage 3 to 5 (non-dialysis dependent) at the third lumbar vertebra region. Other objectives included to assess 1) the agreement between the CT and methods used in clinical practice to evaluate muscle mass; 2) the prevalence of sarcopenia; 3) to identify which substitute method shows better agreement with CT for the diagnosis of sarcopenia. The main findings of the first study showed that midarm muscle circumference (MAMC) and Baumgartner's equation were the methods with higher agreement with CT to assess muscle mass. In addition, the prevalence of sarcopenia varied from 3 to 7%, depending on the method applied to evaluate muscle mass. The second paper of this thesis aimed to evaluate the association between low muscle mass, low muscle strength and sarcopenia with worsening nutritional status and clinical condition, poor quality of life, higher hospitalization risk and mortality in elderly patients on HD. This second study showed that low muscle strength was associated with poorer quality of life and higher hospitalization risk and, sarcopenia, on the other hand, was associated with poorer nutritional status and clinical condition and higher mortality rates. These studies confirm the importance of the assessment of muscle mass in CKD patients, considering their usefulness for the diagnosis of sarcopenia, the prevention of nutritional disturbances and the possibility to apply early therapeutic interventions. In addition, accessible methods in clinical practice, such as anthropometry and predictive equation, showed a good agreement with the gold standard method for the evaluation of muscle mass in CKD patients

A condição inflamatória da doença, o tratamento de diálise, especialmente a hemodiálise (HD) e as alterações na massa muscular relacionadas ao envelhecimento incluem alguns dos fatores que induzem ao aumento do catabolismo proteico. Diversos estudos demonstraram a utilidade das imagens de tomografia computadorizada (TC) como metodo de referencia para a avaliação da massa muscular. Contudo, na população de pacientes com DRC poucos trabalhos foram realizados com a utilização da TC para este fim. Desta forma, o presente estudo tem como objetivo avaliar a massa muscular por métodos de referência e descrever a prevalência de sarcopenia em duas coortes de pacientes com DRC, sendo uma composta por pacientes com DRC não dependentes de diálise e outra composta por pacientes idosos com DRC em tratamento de hemodiálise. Considerando a TC como método de referência, no primeiro artigo desta tese o objetivo foi avaliar a massa muscular de pacientes não dependentes de diálise nos estágios 3 a 5 da DRC através da técnica de TC na região da terceira vértebra lombar. Ademais, estabeleceu-se como objetivos específicos avaliar: 1) a concordância entre a técnica de TC e métodos substitutivos utilizados na prática clínica para avaliação da massa muscular; 2) a prevalência de sarcopenia; 3) identificar qual método substitutivo apresenta maior concordância com a TC para o diagnostico de sarcopenia. Os principais achados do primeiro estudo mostram que a circunferência muscular do braço (CMB) e a equação de Baumgartner foram os métodos com maior concordância com a TC para avaliar a massa muscular. Ademais, notou-se que a prevalência de sarcopenia variou de 3 a 7%, a depender do método utilizado para avaliar a massa muscular. O segundo artigo desta tese teve como objetivo avaliar a associação entre a redução da massa muscular, da força muscular e da sarcopenia com a piora do estado nutricional e condição clínica, redução da qualidade de vida, maior risco de hospitalização e mortalidade em pacientes idosos em HD. Esse segundo estudo apresentou como principais achados que a redução da força esteve associada com pior qualidade de vida e maior risco de hospitalização. Além disso, a condição de sarcopenia associou-se com pior estado nutricional, pior condição clínica e maior taxa de mortalidade. Esses estudos comprovam a importância de se avaliar a massa muscular em pacientes com DRC, considerando sua necessidade para o diagnostico da sarcopenia, na prevenção de alterações do estado nutricional e, pela possibilidade de iniciar intervenções terapêuticas mais precoces.
