Resumo: | Introduction: Tuberculosis (TUBERCULOSE) is an endemic disease that has been present as a public health problem in Brazil throughout the twentieth century, and continues until today. The incidence of TUBERCULOSE cases increases due to the high rate of treatment abandonment, as it is a long treatment, and its main difficulties to obtain a cure, making the disease a major public health problem. The general objective of analyzing the factors associated with the cure of tuberculosis cases reported in the municipality of Mesquita, Rio de Janeiro, from 2007 to 2017. Among the specific objectives are: to describe the sociodemographic variables of tuberculosis cases reported in the municipality. of mosque; To analyze the epidemiological clinical profile of tuberculosis cases in the municipality of Mesquita and to identify factors related to the cure outcome of tuberculosis in Mesquita. Methods: This is a cross-sectional epidemiological study, with data from tuberculosis cases reported in the Notification Disease Information System (SINAN), in the municipality of Mesquita, with emphasis on Primary Care units. The study cutoff was ten years, from 2007 to 2017. The study population will be composed of all tuberculosis cases reported in SINAN. The chi-square test was used to assess differences between proportions. For all analyzes, a significance level of 5% was considered. After adjusting simple logistic models, a stepwise backward manual procedure was performed to estimate the multiple logistic model. For the selection of variables and for comparison between models, the likelihood ratio test (through residual deviance) and the Akaike information criterion (AIC) were used. Finally, in order to verify the quality of the adjusted logistic regression model, we used the Hosmer and Lemeshow statistical test, and graphical analysis of residues. All analyzes were performed using the statistical software R version 3.6.1. Results: of the 1895 cases reported, the following factors were associated with a greater chance of cure of TUBERCULOSE: female gender (OR 1.31; 95% CI 0.928; 1.851); higher education category (OR 6,789; 95% CI 1,937; 2,380) and high school (OR 6,317; 95% CI 2,816; 1,417); HIV negative (OR 1; 95% CI 1,729; 5,072) and the pulmonary form of tuberculosis (OR 0,655 95% CI 0.154; 2,789). Conclusion: social and economic determinants that influence the outcome of treatment were identified, highlighting the need for social support and ensuring access to health services. |

A incidência de casos de TUBERCULOSE aumenta pelo elevado índice de abandono ao tratamento, pois se trata de um tratamento longo, tendo como principais dificuldades para a obtenção da cura, isso faz com que a doença seja um grande problema de saúde pública. O objetivo geral de analisar os fatores associados à cura dos casos de Tuberculose notificados no município de Mesquita, Rio de Janeiro, no período de 2007 a 2017. Dentre os objetivos específicos destacam-se: descrever as variáveis sociodemográficas dos casos de Tuberculose notificados no município de Mesquita; analisar o perfil clínico epidemiológico dos casos de Tuberculose do município de Mesquita e identificar os fatores relacionados com o desfecho cura da Tuberculose em Mesquita. Métodos: trata-se de um estudo epidemiológico do tipo transversal, com dados dos casos de Tuberculose notificados no Sistema de Informação de Agravos de Notificação (SINAN), do município de Mesquita, com ênfase nas unidades de Atenção Básica. O recorte do estudo foi de dez anos, do ano de 2007 a 2017. A população do estudo será composta por todos os casos de Tuberculose notificados no SINAN. Foi utilizado o teste de χ2 (qui-quadrado) para avaliar diferenças entre proporções. Para todas as análises foi considerado um nível de significância de 5%. Após o ajuste de modelos logísticos simples, realizou-se um procedimento stepwisebackward manual para estimação do modelo logístico múltiplo. Utilizou-se para seleção de variáveis e para comparação entre modelos o teste de razão de verossimilhança (por meio da deviance residual) e o critério de informação de Akaike (AIC). Por fim, a fim de verificar a qualidade do modelo de regressão logístico ajustado, foi utilizado o teste estatístico de Hosmer e Lemeshow, e análise gráfica de resíduos. Todas as análises foram realizadas utilizando o software estatístico R versão 3.6.1. Resultados: dos 1895 casos notificados, revelaram-se fatores associados a maior chance de cura da TUBERCULOSE: o sexo feminino (OR 1,31; IC95% 0,928;1,851); escolaridade categoria do ensino superior (OR 6,789; IC95% 1,937;2,380) e médio (OR 6,317; IC95% 2,816;1,417); HIV negativos (OR 1; IC95%1,729; 5,072) e a forma pulmonar da Tuberculose (OR 0,655 IC95%0,154; 2,789).
