Resumo: | The cytotechnologists perform the scrutiny of cytological samples using the optical microscope. They are often affected by Work-Related Musculoskeletal Disorders (WMSDs) due to excessive force, repetitive movements, bad posture, static posture or vibration during use of the microscope. In Brazil, there is no legal requirement for the number of slides that cytotechnologists must see throughout their workday. In this context, we sought to identify and quantify the repetitive activities performed by cytotechnologists of the Integrated Section of Technology in Cytopathology (SITEC) of the Division of Pathology (DIPAT) of the National Cancer Institute José Alencar Gomes da Silva (INCA) in the screening of cervicovaginal cytological preparations, to measure musculoskeletal symptoms and to seek opportunities for improvements in the work process, aiming at reducing injuries to professionals. We applied the Nordic Osteomuscular Symptoms Questionnaire for Cytotechnologists, we analyzed the disease records in the INCA Worker Health Division (DISAT) during the period from January 2016 to December 2017 and performed the counting of the reading time and movements in the charriot, fine focus knob, coarse focus knob and in the revolving nosepiece of the optical microscope was performed for five cytotechnologists during the reading of three conventional cytology slides and three liquid base cytology slides. The data obtained for analysis were organized into tables using the Statistical Package for the Social Sciences (SPSS). Based on the data from the questionnaire, 34.4% of the cytotechnologists reported withdrawal from work by WMSD and the main complaints were in the region of the cervical and lumbar spine. According to DISAT records, the main reasons for absenteeism among SITEC professionals were diseases categorized in the musculoskeletal and connective tissue diseases group (25%). There was a statistically significant relationship between Body Mass Index (BMI) and WMSDs absenteeism. We obtained an average of five hundred (500) touches and three (3) dorsal revolutions per slides by suming the movements in the charriot, fine focus knob, coarse focus knob, revolving nosepiece and dorsal rotation among all cytotechnologists, in the six cases. The average reading time of the conventional cytology slides was 278.4 seconds, while the mean cytology slides were 285.5 seconds. Long hours using the optic microscope and or improper posture during screening can generate musculoskeletal problems in cytotechnologists. Ergonomics awareness is critical for risk identification and planning for preventive measures. The evaluation of the quantitative of touches in the microscope is necessary for the definition of the workload minimizing the musculoskeletal discomforts associated with repetitive movements. The introduction of new technologies such as liquid-based cytology, which allows the automation of stages of its processing, should contribute to propose improvements on the workload of these professionals. |

São frequentemente acometidos por Distúrbios Osteomusculares Relacionados ao Trabalho (DORT) devido a força excessiva, movimentos repetitivos, postura inadequada, postura estática ou vibração durante a utilização do microscópio. No Brasil, não há uma determinação legal quanto ao número de lâminas que os citotecnologistas possam ler ao longo da sua jornada de trabalho. Neste contexto, buscamos identificar e quantificar as atividades repetitivas realizadas pelos citotecnologistas da Seção Integrada de Tecnologia em Citopatologia (SITEC) da Divisão de Patologia (DIPAT) do Instituto Nacional de Câncer José Alencar Gomes da Silva (INCA) no escrutínio de preparações citológicas cervicovaginais, mensurar os sintomas musculoesqueléticos e buscar oportunidades de melhorias no processo de trabalho, visando à redução de danos aos profissionais. Para isso, aplicamos o Questionário Nórdico de Sintomas Osteomusculares para Citotecnologistas, analisamos os registros de doença na Divisão de Saúde do Trabalhador (DISAT) do INCA durante o período de janeiro de 2016 a dezembro de 2017 e realizamos a contagem do tempo de leitura e dos movimentos no charriot, micrométrico, macrométrico e revólver do microscópio óptico realizados por cinco citotecnologistas durante a leitura de três lâminas de citologia convencional e três lâminas de citologia de base líquida. Os dados obtidos para análise foram organizados em tabelas utilizando-se o programa Statistical Package for the Social Sciences (SPSS). Com base nos dados do questionário, 34,4% dos citotecnologistas relataram afastamento do trabalho por DORT e as principais queixas foram na região da coluna cervical e lombar. De acordo com os registros da DISAT os principais motivos de absenteísmo dos profissionais que trabalham na SITEC foram categorizados no grupo de doenças do sistema osteomuscular e do tecido conjuntivo (25%). Foi observada relação estatisticamente significativa entre o Índice de Massa Corporal (IMC) e o absenteísmo por DORT. Ao somar os movimentos no charriot, micrométrico/macrométrico, revólver e rotação dorsal entre todos os citotecnologistas, nos seis casos, obtivemos uma média de quinhentos (500) toques e três (3) rotações dorsais por lâmina. A média do tempo de leitura das lâminas de citologia convencional foi de 278,4 segundos, já a média das lâminas de citologia em base líquida foi de 285,5 segundos. Longas horas utilizando o microscópio óptico e/ou postura inadequada durante o screening podem gerar problemas musculoesqueléticos em citotecnologistas. A conscientização sobre a ergonomia é fundamental para identificação dos riscos e planejamento sobre medidas preventivas. A avaliação do quantitativo de toques no microscópio é necessária para a definição da carga de trabalho minimizando os desconfortos osteomusculares associados a movimentos repetitivos. The evaluation of the quantitative of touches in the microscope is necessary for the definition of the workload minimizing the musculoskeletal discomforts associated with repetitive movements. The introduction of new technologies such as liquid-based cytology, which allows the automation of stages of its processing, should contribute to propose improvements on the workload of these professionals. |
