The article aims to present the mental health assistance for adolescents in psychoactive substance abuse in Fortaleza, after the municipal health department determined, in 2021, that until the age of 16 the monitoring would be the responsibility of the CAPSi and that, from the age of 16, the treatment would take place at the CAPSad. The study was exploratory and descriptive. Semi-structured interviews and conversation circles were conducted with professionals from two CAPSad and one children's hospital. It was observed that adolescents have not been able to reach the psychosocial care centers, but are admitted to the hospital. The professionals point out as elements for the non-arrival of adolescents at the CAPS: the lack of preparation and management of the workers to deal with this public, the inadequacy of the physical space of the services, the absence of an offer of activities for the adolescents, the difficulties in carrying out actions in the territory and in intersectoral networks, the difficulties of young people moving through factional territories, the lack of perception of substance use as a problem by adolescents and the central place occupied by medications and hospitalizations.
