Dependência funcional e a necessidade de fisioterapia na fase aguda do acidente vascular cerebral

Resumo: Introduction: Stroke is an important cause of functional impairment and patients require physical therapy from the acute phase. Objetives: The aim of the study was to evaluate the functional dependency and prescription and no realization of Physiotherapy at a public hospital in the northeast, in acute stroke. Methodology: We conducted a follow-up study in which 305 patients participated (158 women and 147 men), 23 to 99 years, assessed using an instrument based epidemiological (Step 1), together with the Modified Rankin Scale in order to record the functional dependence in Activities of Daily Living in the 3rd, 10th and 28th day of the stroke, and a questionnaire to verify the need of physical therapy for patients. Data were analyzed by chi-square test. Results: Significant difference was found between the degree of dependence and independence for the 6-valued functions (p = 0.0001), and patients are dependent in all activities. The item "dress up" was what showed greater dependence (79.7%) and the item "continence" was to lower dependence (53.1%) (p<0.0001). However, the patients evaluated, 75% did not undergo physical therapy during hospitalization. After discharge, 82.3% of patients did not seek any service not started physical therapy. Conclusion: Thus, the findings indicate that the situation is more serious than you think, because despite the functional dependence found, few patients underwent physical therapy. We suggest the need for more services and Physiotherapy professionals to hospital performance since the beginning of the disease.

Objetivos: Avaliar a dependência funcional e se ocorre prescrição e realização da Fisioterapia em um hospital público do nordeste brasileiro, na fase aguda do AVC. Metodologia: Foi realizado um estudo de seguimento, no qual participaram 305 pacientes (158 mulheres e 147 homens), de 23 a 99 anos, avaliados através de um instrumento de base epidemiológica (Step 1), junto com a Escala de Rankin Modificada, a fim de registrar a dependência funcional nas AVDs no 3º, 10º e 28º dia do AVC, além de um questionário para verificar a necessidade da Fisioterapia para os pacientes. Os dados foram analisados pelo teste Qui-quadrado. Resultados: Foi encontrada diferença significativa entre os graus de dependência e independência para as 6 funções avaliadas (p= 0,0001), sendo os pacientes dependentes em todas as atividades. O item “vestir-se” foi o que mostrou maior dependência (79,7%) e o item “continência” foi o de menor dependência (53,1%) (p<0,0001). Entretanto, dos pacientes avaliados, 75% não realizaram Fisioterapia durante a internação. Após a alta, 82,3% dos pacientes não procuraram algum serviço e não iniciaram tratamento fisioterapêutico. Conclusãos: Dessa forma, os achados indicam que a situação é mais grave do que se imagina, pois apesar da dependência funcional encontrada, poucos pacientes fizeram tratamento fisioterapêutico.
