Resumo: Over the years, the world is experiencing a process of demographic and epidemiological transition. Chronic non-communicable diseases (NCDs) have become part of everyday life for most of the population, especially the elderly. Leading them to a progressive functional decline, which precedes death. At a certain point, it is necessary to transition from healing care to comfort care. Palliative Care does not aim to accelerate or delay death but to promote life, accepting death as a natural and inevitable process. This work aimed to investigate in the literature the challenges in the process of transition to palliative care for elderly patients. This was an integrative literature review, data collection took place between January and February 2023, with a survey of publications through the following databases: Medline/Pubmed, EMBASE, Scopus, and Web of Science. After searching the databases, a total of 572 articles were selected, of which 216 were from Medline/Pubmed, 125 from SCOPUS, 79 from WEB OF SCIENCE, 152 from EMBASE. After this phase, 217 duplicate articles were excluded, leaving a total of 355 articles for title and abstract reading. After pre-selection by reading the title and abstract, 300 articles were excluded, leaving 55 articles for reading in full. After reading them in full, 4 articles were selected. Several challenges related to the transition to palliative care for elderly patients were identified. The most cited were the limited understanding of palliative care related to the patient and family but also to health professionals, ineffective communication, overlapping roles and the difficulty of timely referral to palliative care. It is possible to perceive the contemporaneity of the Palliative Care theme and its challenges, this is reflected in the selected articles, it is a current theme but there is little research on the subject. No article was found in the searched databases of Brazilian nationality, showing the need for further studies on the subject. The most recent study was carried out in 2020, showing a long period of time where there was no research on the subject. When challenges are identified, it is possible to correct the problems that hinder this smooth and timely transition.

As doenças crônicas não transmissíveis (DCNT) se tornaram parte do cotidiano da maior parte da população, principalmente dos idosos. Levando-os a um declínio funcional progressivo, que antecede o óbito. Sendo necessário em um determinado momento a transição dos cuidados de cura para os cuidados de conforto. Os Cuidados Paliativos não têm como objetivo acelerar ou retardar a morte mas promover a vida, aceitando a morte como processo natural e inevitável. Esse trabalho teve como objetivo investigar na literatura os desafios no processo de transição aos cuidados paliativos do paciente idoso. Tratou-se de uma revisão integrativa de literatura, a coleta de dados ocorreu entre janeiro e fevereiro de 2023, realizando-se o levantamento de publicações através das seguintes bases de dados: Medline/Pubmed, EMBASE, Scopus, e Web of Science. Após as buscas nas bases de dados, foram selecionados 572 artigos no total, destes 216 artigos da Medline/Pubmed, 125 da SCOPUS, 79 da WEB OF SCIENCE, 152 da EMBASE. Após essa fase, foram excluídos 217 artigos duplicados, restando um total de 355 artigos para leitura de título e resumo. Após a pré-seleção feita com leitura do título e do resumo, 300 artigos foram excluídos, restando 55 artigos para leitura na íntegra. Após a leitura na íntegra, 4 artigos foram selecionados. Foram identificados vários desafios relacionados à transição aos cuidados paliativos do paciente idoso. Os mais citados foram a compreensão limitada sobre cuidados paliativos relacionados ao paciente e família mas também aos profissionais da área da saúde, a comunicação ineficaz, a sobreposição de papéis e a dificuldade do encaminhamento oportuno aos cuidados paliativos. É possível perceber a contemporaneidade do tema Cuidados Paliativos e seus desafios, isso se reflete nos artigos selecionados, é um tema atual mas há poucas pesquisas sobre o assunto. Não foi encontrado nenhum artigo nas bases de dados pesquisadas de nacionalidade brasileira, mostrando a necessidade de mais estudos sobre o assunto. O estudo mais recente foi realizado em 2020, evidenciando um grande espaço de tempo onde não houve pesquisas sobre o assunto.
