Modelo acadêmico de ensino teórico-prático em vídeo cirurgia, por meio de novo simulador real de cavidade abdominal

Resumo: Introduction -The Surgical Skills Laboratory has increasing its relevance and importance in this century of technology and information, in medical schools, especially in the process of teaching and learning, and in training activities of continuing education programs, beyond research, testing new techniques and contributing to the evolution of surgery. ObjectiveThis study aims to validate the theoretical and practical teaching model, applied with the medical students, putting the EndoSuture Training Box (ESTB), developed by the author, into the subjects program, as a learning and training tool, which stimulates the development of psychomotor abilities in video surgery, to consolidate the progression skills until reach satisfactory proficiency. Material and Method - This investigation was developed at the Surgical Skills Laboratory, with conventional surgical and endoscopic material, simulators 3-DMed® and EndoSuture Training Box, besides questionnaires in Likert scale scores. Three homogeneous groups from different subjects, formed by students were distributed to perform tasks with different levels of difficulty to evaluate and to validate this model of teaching. The fourth group, formed by surgeons with different levels of proficient, executed tasks to validates the technical and technological profile of the ESTB simulator. According to the performance, they were stratified on Skills and Proficiency Suture Scale on Video Surgery. Results - As main result, the theoretical and practical teaching model at the Surgical Skills Laboratory was well evaluated and validated as a learning tool and can remain within the curriculum of medical education platform. The progression of skill training steps from the manual node to extra instrumental and intra-corporeal were experienced in the theoretical practical lessons of the surgical clinic discipline, with the Surgical Skills Laboratory. Discussion - The preparation of the surgical point was an important component in the students training and can be systematized and standardized to the absorption of surgery video concepts, acquired in realistic simulation with safety and then transferred to the operation room.Conclusion - The model of theoretical and practical teaching of surgery developed at the Surgical Skills Laboratory has been validated as a tool within the curriculum platform of the medical degree. The ESTB real simulator was well evaluated and validated by surgeons, accordingly, can be insert into the context of the Surgical Skills Laboratory, as a training tool in surgery.

Objetivo - Validar o modelo teórico-prático de ensino em vídeo cirurgia, aplicado junto aos graduandos do Curso de Medicina da Universidade Estadual do Ceará, inserindo na plataforma curricular, por meio de um novo simulador real de cavidade abdominal, EndoSuture Trainning Box (ESTB), desenvolvido pelo autor, como instrumento de aprendizado, treinamento, progressão de habilidade psicomotoras em vídeo cirurgia, para consolidar a progressão e a manutenção da proficiência emvideo cirurgia.. Material e Método - A pesquisa foi executada no ambiente do Laboratório de Habilidades Cirúrgicas (LHC), do Hospital Geral Cesar Cals, hospital-escola terciário da rede pública da Secretaria de Saude do Estado do Ceará - Brasil com material cirúrgico convencional e endoscópico, simuladores 3-DMed® e ESTB, além de questionários em escala de Likert. Três grupos homogêneos formados por alunos de disciplinas diferentes (GI - alunos do quinto semestre - disciplina de Base da Técnica Cirúrgica; GII, alunos do sétimo semestre - disciplina de Clínica Cirúrgica: e GIII alunos que cumpriram, previamente, as duas disciplinas), foram distribuídos para executar tarefas de diferentes progressões de dificuldades para validar o modelo de ensino e o quarto grupo (GIV, constituído por cirurgiões com vários níveis de proficiência, que executou tarefas para avaliar e validar o perfil técnico e tecnológico do simulador ESTB e, conforme o desempenho das tarefas, estes foram estratificados na Escala de Habilidades e Proficiência de Sutura em Vídeo Cirurgia. Resultados Os resultados mostraram que o modelo teórico-prático de ensino no LHC foi bem avaliado e validado como instrumento de aprendizado e pode permanecer no contexto da plataforma curricular da graduação médica. A progressão da habilidade do treinamento desde o nó manual, instrumental extra e intra-corpóreo foram etapas vivenciadas nas aulas teórico-práticas da disciplina de clínica cirúrgica, junto ao LHC. Discussão - A confecção do ponto cirúrgico foi componente importante no treinamento do aluno, podendo ser metodizado e padronizado para a absorção dos conceitos de vídeo cirurgia adquiridos na simulação realística, com segurança e depois transferido para a sala de operação. Conclusão O modelo de ensino teórico-prático de cirurgia desenvolvido no LHC foi validado como instrumento dentro da plataforma curricular da graduação de médica. O simulador ESTB foi bem avaliado e validado pelos cirurgiões podendo ser inserido no contexto do LHC, como instrumento de treinamento em cirurgia.
