Associação entre câncer de mama e uso de contraceptivos orais de mulheres em idade fértil

Resumo: The association between oral contraceptives (ACO) and breast cancer has been debated for decades, without consensus. In Brazil, breast cancer mortality rates remain high, most likely because the disease is still diagnosed in advanced stages. For the 2018-2019 biennium, 59,700 new cases of breast cancer and 15,593 deaths are estimated, 187 men and 15,443 women in Brazil. Birth control pills are among the most popular contraceptive methods, but fear of cancer and cardiovascular disease overshadows their continued use in women. This study aimed to investigate whether the use of oral contraceptives before the age of 20 is a determining factor for the onset of cancer. The approach of this study was epidemiological, observational, analytical case-control. The studied population was searched from hospital records, from the SAME filing sector of MEAC from August 2015 to October 2018, the study population was 612 hospital records, and 292 unfilled records were excluded. A standardized form was used. Data analyzes were presented in tables, using categorical variables, Pearson's chi-square test and Fisher's exact test were used. A significance level of 5% was adopted. Statistical analyzes were performed using the statistical program JAMOVI and Microsoft Excel 2016. It was found that the use of OCA associated with breast cancer, p <0.001, with the highest prevalence of OCA use among cases of neoplasia (100%) 76 times more likely to have cancer, OR (95% CI) and family history (25) 22.5% with a significant value (p = 0.001). Regarding the immunohistochemical variable, it presented a negative PR (47) 52.2% with a significant value (p = 0.001). Thus, it is concluded that more studies are needed to better understand the role of oral contraceptives as a triggering factor for breast cancer. Keywords: Breast Cancer. Oral contraceptives. No Brasil, as taxas de mortalidade por câncer de mama continuam elevadas, muito provavelmente porque a doença ainda é diagnosticada em estágios avançados. Estimam-se para o biênio 2018-2019, 59.700 novos casos de câncer de mama e 15.593 óbitos, sendo 187 homens e 15.443 mulheres no Brasil. As pílulas anticoncepcionais estão entre os métodos contraceptivos mais populares, mas o medo do câncer e doenças cardiovasculares ofusca seu uso contínuo em mulheres. Este estudo teve como objetivo investigar se o uso de contraceptivo oral antes dos 20 anos é fator determinante para o aparecimento do câncer. A Abordagem desse estudo foi epidemiológico, observacional, analítico do tipo caso-controle. A população estudada foi pesquisada de prontuários hospitalares, provenientes do setor de arquivamento SAME da MEAC no período de agosto de 2015 a outubro de 2018, a população do estudo foi de 612 prontuários hospitalares, e foram excluídos 292 prontuários não preenchidos. Foi utilizado um formulário padronizado. As análises dos dados foram apresentadas em tabelas, utilizando-se as variáveis categóricas utilizou-se o teste qui-quadrado de Pearson e o teste exato de Fisher. Adotou-se um nível de significância de 5%. As análises estatísticas foram realizadas utilizando o programa estatístico JAMOVI e Microsoft Excel 2016. Constatou-se que o uso de ACO associado com câncer de mama, p<0,001, sendo a maior prevalência de uso de ACO dentre os casos de neoplasia (100%) com chance 76 vezes maior de ter câncer, OR (IC95%) e antecedentes familiares (25) 22,5% com valor significante (p=0,001). Com relação a variável imuno-histoquímica apresentou RP negativo (47) 52,2% com valor significante (p= 0,001). Dessa forma, conclui-se que mais estudos são necessários, para uma melhor compreensão do papel dos contraceptivos orais como fator desencadeante do câncer de mama. Palavras-chave: Câncer de mama.
