Teoria de situação específica e análise de conteúdo do diagnóstico de enfermagem nutrição desequilibrada: menor do que as necessidades corporais em crianças com câncer

Resumo: The safe use of nursing diagnoses in clinical practice requires them to be trusted, highlighting the importance of diagnostic validation for review and try them. Thus, the aim of this study was to develop two phases of diagnostic validation to achieve the evidence of theoretical and content validity of nursing diagnosis Imbalanced nutrition: less than body requirements in children with cancer. These phases included the development of Situational Specific Theory (SST) and the content analysis for judges. The first phase, development between April of 2017 to July of 2018, was based on pre-existing of nursing diagnosis in a study contained in the NANDA International taxonomy, in the subcomponent Nutrition described in Roy’s Adaptation Model, and in the literature review. The first phase happened according to the following steps: approach definition to building SST, conceptual models’ definitions to be analyzed, key-concepts definitions, development of pictogram, development of propositions, and causal relations between the elements and practice evidence. New essentials attributes were identified. From that, 26 antecedents’ clinical factors (ACF) and 22 consequents’ clinical factors (CCF) were identified, building conceptual and empirical definitions. The cause-effect pictogram was development after the analysis of these elements. Additionally, ten propositions were made, and causal effects were established. Thus, a considerable difference between elements of SST and NANDA-I was verified. In NANDA-I the diagnosis definition is restricted to the Ingestion stage, has only seven ACF, and the 22 CCF were different. The second phase was content validity by judges and aiming to verify the appropriateness of the diagnostic label, the relevance of ACF and CCF for diagnosis, and the clarity and precision of conceptual and empirical definitions. For this phase, the Predictive diversity theorem was used. 23 judges were selected, in based on practice and academic experience, and they were classified in five levels of expertise. The data collection was between February and June of 2019. The pseudo-median was used weighted by the level of judges’ expertise, adopting 0.9 for the Content Validation Index as a cutoff point. Regarding the adequacy of the new diagnostic label, 78.26% of the judges judged the label and definition listed for SST to be more appropriate. Of the 22 CCF analyzed, five were not considered relevant, they are: Apathy; Edema of lower limbs; Petechiae, purples or bruises; Fracture; and Poor Perceptual Organization. The CCF Dry and brittle hair and Pale mucous membranes did not show, respectively, precision of conceptual definition and precision of operational definition. Regarding the ACF, one factor was not submitted to analysis, thus, of the 25 analyzed, four were not considered relevant, they are: Non-acceptance of the disease, 11 Loneliness, Disturbed interpersonal relationships and Dysgeusia. The latter, it is recommended to keep it due to evidence in robust studies. The conceptual definitions for Nausea/vomiting and Inadequate eating pattern did not present, respectively, clarity and precision. Regarding the operational definitions of the ACF Anorexia and Nausea / vomiting were not accurate. The 17 CCF deemed relevant had high CVI-G, moderate agreement between judges and high internal consistency. This also happened to the 21 ACF deemed relevant. Therefore, changes were made to the definitions as suggested by the judges. It is concluded that, according to the methods employed, the present study provided the refinement and improvement of the components of Imbalanced nutrition: less than body requirements in the population of children with cancer, composed, until this stage, by new essential attributes, 22 ACF and 17 CCF. Thus, it helps to identify the presence of the diagnosis in that population with more precision.

Assim, o objetivo deste estudo foi desenvolver duas etapas do processo de validação de diagnósticos de enfermagem com o intuito de obter evidências de validade teórica e de conteúdo do diagnóstico Nutrição desequilibrada: menor do que as necessidades corporais no contexto de crianças com câncer. Essas etapas incluíram o desenvolvimento de uma Teoria de Situação Específica (TSE) e o processo de avaliação do conteúdo diagnóstico por juízes. A primeira etapa baseou-se na estrutura pré-existente do diagnóstico de enfermagem em questão contido na taxonomia da NANDA Internacional, no subcomponente Nutrição do modo fisiológico descrito no Modelo de Adaptação de Roy e numa revisão integrativa da literatura. Esta primeira etapa, desenvolvida de abril de 2017 a julho de 2018, ocorreu conforme as etapas: definição da abordagem para construir a TSE, definição de modelos conceituais a serem analisados, definição de conceitos chaves, desenvolvimento de um diagrama pictórico, construção de proposições e estabelecimento de relações causais entre os elementos e evidências para a prática. Como resultado novos atributos essenciais foram elencados. A partir disso, identificou-se 26 fatores clínicos antecedentes (FCA) e 22 fatores clínicos consequentes (FCC) correspondentes ao diagnóstico em estudo, sendo elaboradas definições conceituais e operacionais. A análise desses conceitos permitiram o desenvolvimento do diagrama do tipo causa e efeito, construção de 10 proposições e estabelecimento de efeitos causais. Assim, verificou-se uma diferença considerável entre os elementos elencados para TSE e os incluídos na NANDA internacional. Nesta, a definição do diagnóstico restringe-se à etapa de ingestão, são apenas sete os FCA e os 22 FCC são diferentes. A segunda etapa consistiu na avaliação do conteúdo diagnóstico por juízes e teve por finalidade verificar a adequação da etiqueta diagnóstica, a relevância dos FCA e FCC para o estabelecimento do diagnóstico e a clareza e precisão das definições conceituais e operacionais determinadas para TSE. Para esta etapa, utilizou-se como referencial metodológico o teorema da diversidade preditiva. A seleção dos 23 juízes baseou-se em parâmetros pré-estabelecidos, que levaram em conta experiência acadêmica e prática, classificando-os em cinco níveis de expertise. A coleta de dados ocorreu nos meses de fevereiro a junho de 2019. A medida de avaliação utilizada foi a pseudomediana das avaliações realizadas pelos juízes, ponderadas pelo nível de expertise de cada avaliador, adotando-se como ponto de corte um valor de 0,9 para o 9 Índice de Validação de Conteúdo, a fim de considerar a adequação de cada item avaliado individualmente. Quanto à adequação da nova etiqueta diagnóstica, 78,26% das juízas julgaram ser mais adequada a etiqueta e a definição elencadas para a TSE. Dos 22 FCC analisados, cinco não foram considerados relevantes, são eles: Apatia; Edema de membros inferiores; Petéquias, púrpuras ou equimoses; Fratura; e Organização Perceptual Insatisfatória. Os FCC Cabelo seco e quebradiço e Membranas mucosas pálidas não apresentaram, respectivamente, precisão da definição conceitual e precisão da definição operacional. Em relação aos FCA, um fator não foi submetido a análise, assim, dos 25 analisados, quatro não foram considerados relevantes, são eles: Não aceitação da doença, Solidão, Relações interpessoais perturbadas e Disgeusia. Este último, recomenda-se mantê-lo devido evidências em estudos robustos. As definições conceituais dos FCA Náuseas/vômitos e Padrão alimentar inadequado não apresentaram, respectivamente, clareza e precisão. Em relação às definições operacionais do FCA, Anorexia e Náuseas/vômitos não apresentaram precisão. Os 17 FCC julgados relevantes apresentaram IVC-G alta, concordância entre os juízes moderada e a consistência interna alta. O que também ocorreu para os 21 FCA julgados relevantes. Diante disso, foram feitas alterações nas definições conforme sugestões das juízas. Conclui-se que, de acordo com os métodos empregados, o presente estudo proporcionou o refinamento e aperfeiçoamento dos componentes do diagnóstico em estudo na população de crianças com câncer, composto, até esta etapa, por novos atributos essenciais, 22 FCA e 17 FCC.
